Job summary
This is an exciting role which will contribute to
reshaping neighbourhood care for our populations. You will be working as part of
a well-supported, large and expanding multi-disciplinary PCN team, across a
diverse population. The role will operate across a neighbourhood of approximately
four to five practices, providing specialist resource and supporting delivery
of PCN service objectives.
Main duties of the job
The post-holder
-
Under GP supervision, will provide care for the
presenting patient from initial history taking and clinical assessment through
to the diagnosis, treatment and evaluation of care.

Will demonstrate critical thinking in the clinical
decision-making process, including assessment and diagnostic skills, leading to
the delivery of safe care for all patients within the service.

Will work collaboratively with the healthcare team to
meet the needs of the patients, supporting the delivery of policy and
procedures.

will provide a holistic and clinical service, with
support from GPs, implementing agreed management plans and following approved
protocols as appropriate.

opportunities for innovation and development of the service offer.
About us
Our South Southwark PCN is made up
of all 18 GP Practices in Camberwell, Peckham and Dulwich, approximately
144,000 patients, supported by their GP Federation,
Improving Health (IHL). We are a large PCN with 20
staff in post to date and expanding significantly over the coming year. We are
committed to supporting and developing our staff, knowing this is crucial to
our success. We have robust systems in
place, including clinical supervision and peer support, and will continue to build
on this as we expand our PCN team.

- DBS clearance
- Current and valid certification or re-certification by the Physician Associate National Examination
