Job summary

The Alcohol & Drug Service (ADS) are a registered charity who have been transforming the lives of people affected by drugs and alcohol misuse for over 35 years.

East Riding Partnership (ERP) is a partnership between ADS and Humber Teaching NHS trust. It has three hubs, Hull, Bridlington and Goole. It is multi-disciplinary team led by a consultant and provides treatment services in the community. The services include aftercare, young people, community rehabilitation programme, a specialist supporting image and performing enhancing drugs/steroids and a team supporting service users and cares through a team of volunteers and peer mentors.

Main duties of the job

Undertake comprehensive assessment recovery star in order to assess the eligibility and suitability of a service user in relation to the East Riding Partnership. Where the service user is assessed as suitable and eligible, utilise this assessment to create an individually tailored package of care for the service user, in line with statutory requirements where appropriate, to facilitate positive change for the service user through the delivery of the care packaged including, where appropriate, psychosocial, pharmacological and psychological interventions

Conduct risk/need assessments and implement risk management/care plans in line with consortium policy and procedures. Develop packages of care to meet the treatment needs of low risk/non-complex service users, liaising with other health and social care professionals where necessary. Facilitate risk/need reviews as per consortium requirements

Function as part of a multi-disciplinary team, contributing to team meetings as required.

Case-manage a caseload of low risk/non-complex Substance Misusing service users in line with a recovery focused model, utilising 1-2-1 and group-work sessions as appropriate

The majority of our funding comes through contracts with statutory commissioners such as Local Authorities and the NHS, but we receive valuable income from charitable grants and individual fundraising efforts which supports many of our most innovative services. We are governed by a Board of Trustees.

But, even though weve been transforming the lives of people affected by drugs or alcohol misuse for over 30 years, some people may not have heard of us. This is because we always work in partnership and often deliver our services and projects which have their own names/identities.

Main Responsibilities:

1. Principal Accountabilities

Undertake comprehensive assessment recovery star in order to assess the eligibility and suitability of a service user in relation to the East Riding Partnership. Where the service user is assessed as suitable and eligible, utilise this assessment to create an individually tailored package of care for the service user, in line with statutory requirements where appropriate, to facilitate positive change for the service user through the delivery of the care packaged including, where appropriate, psychosocial, pharmacological and psychological interventions

Conduct risk/need assessments and implement risk management/care plans in line with consortium policy and procedures. Develop packages of care to meet the treatment needs of low risk/non-complex service users, liaising with other health and social care professionals where necessary. Ensure all possible steps are taken to account for personal safety of staff and service users, and escalate areas of concern where appropriate

Contribute to reports and participate in case conferences and multi-disciplinary team meetings and any other appropriate care planning meetings in relation to low risk/non-complex service users

Contribute to reports and participate in multi-agency meetings in line with requirements of service user engagement with the service

Take personal responsibility for maintaining practice in line with developments and new initiatives

Undertake screening for Drug and Alcohol use

Collate statistical information as required to meet the needs of key stakeholders, both internally and externally

Proactively encourage drug and alcohol users to access substance misuse services, housing provision and where appropriate, social support services; to promote and facilitate the harm reduction model of treatment provision in all aspects of work, to enable substance users to adopt safer practices and healthier lifestyles

To assist service users, where appropriate to address their offending behaviour and to provide support to other criminal justice interventions such as Probation, IOM etc

To maintain and update excellent service user records

Work with other health and social care and criminal justice providers as required

To communicate concerns regarding service users children and safeguarding concerns

To report as soon as practicable any incidents, complaints or grievances

2. Safety

To ensure all possible steps are being taken to account for personal safety of both staff and service users during working hours To make the line manager aware of any potential difficulties in the area of personal safety

Adhere to all Alcohol & Drug Service Health and Safety policies and procedures and ensure their own safety and the safety of colleagues and service users

To be aware of the need for security and regard to locking windows, doors etc and prompt reporting of any works/repairs to the line manager

3. Supervision and Training

Keep personally up to date with developments and new initiatives

Participate in regular supervision sessions with the line manager

To participate in the appraisal process and work with the line manager to develop a personal training and development plan

Attend and participate in training sessions as required both as trainer and trainee

Provide an effective learning environment for all learners

4. Safeguarding

The Alcohol & Drug Service is committed to safeguarding and promoting the welfare of children, young people (anyone under the age of 18) and adults. It expects all staff, volunteers and students to share this commitment and act in such a way that safeguards and promotes the health and wellbeing of children, young people and adults.
