This is an exciting
opportunity to become part of our Community Frailty Hub team as a Therapy
Assistant, providing vital support to Physiotherapists, Occupational
Therapists, and Nurses. Together, we deliver timely, safe, and compassionate
care to our patients.
In this role, you will work
closely with a multidisciplinary team of healthcare professionals, supporting
patients individual needs while promoting health, wellbeing, and independence.

Main duties of the job
To provide therapy
interventions within an agreed scope of practice following assessment by
qualified Occupational Therapy and Physiotherapy staff.
To be responsible for a
caseload of patients working to agreed treatment objectives and to assist Physiotherapists
and Occupational Therapists in the treatment of patients who require more than
one therapist for safe handling, complex rehabilitation
To support with the
collection, delivery and review of equipment prescribed by the Physiotherapist
or Occupational Therapist.
To support patients with
personal cares including hygiene needs, nutrition and hydration and medication
administration.
To maintain patient
records and recording data in an accurate and timely manner.
To support the therapy
team in the administrative duties, particularly those required to ensure
efficient delivery of patient care.
To provide updates to
the Therapy team and other members of the multi-disciplinary team (MDT) and
participate in the planning and delivery of high quality of care for our
patients.
To be responsible for
prioritising and planning own timetable, acknowledging the need for
prioritisation, escalation and the ability to be flexible.
To review caseload on a
regular basis with senior colleagues and report back appropriately any
concerns, if patient goals are not achieved or if new information becomes available
which may impact on the intervention provided.

We deliver Urgent Care Services in collaboration with the hospital and GP practices in York, Scarborough, Selby, Malton and Whitby. We also provide community based frailty care which focuses on treating people in the community where we can. We also aim to shift services into the community where possible, for example our Community Diagnostic Centre runs in partnership with the Acute Trust.
