This is a unique opportunity to train as an ACP while gaining practical experience across a range of urgent care settings.

Who Should Apply?

This opportunity is ideal for:

  • Registered Nurses, Paramedics, or Physiotherapists with a strong interest in urgent or primary care and who are able to prescribe
  • Practitioners with at least three years of post-registration or comparable experience
  • Practitioners ready to work across professional boundaries and contribute to a dynamic, multidisciplinary team

Main duties of the job

  • As a Trainee ACP, you will:

    • Deliver patient care in urgent and unscheduled settings under direct or indirect clinical supervision
    • Ideally already practiced in independent prescribing, however this can be incorporated into the ACP course, you must meet the entry requirements for this module as prescribing is a requirement of the role.
    • Complete the MSc in Advanced Clinical Practice at the University of York (part-time)
    • Rotate through diverse clinical settings such as:
      • Clinical Assessment Service (CAS)
      • Urgent Treatment Centres (UTCs)
      • Same Day Urgent Care Clinics
      • Out-of-Hours (OOH) GP services

We deliver Urgent Care Services in collaboration with the hospital and GP practices in York, Scarborough, Selby, Malton and Whitby. We provide community based frailty care which focuses on treating people in the community where we can. We aim to shift services into the community where possible, for example our Community Diagnostic Centre runs in partnership with the Acute Trust.

The post holder will build evidence across all four pillars of the advanced clinical practice framework. The post holder will provide clinical leadership and support to the practitioner workforce, nursing and allied health teams to improve the patient experience, supporting the advanced clinical practitioner, service lead and medical and nursing team with day-to-day clinical activity (to be determined according to need of specialist area). Supporting the designated workspace areas to deliver consistent high quality clinical care and supporting with both innovation and on-going clinical care provision, ensuring patients are in the right place at the right time with the right care.

With support manage the complete clinical care for patients, to enhance patient care and improve patient pathways in line with national standards.

Under supervision and with support will complete comprehensive physical and mental health assessments, providing differential diagnoses and deciding on the appropriate treatment based on clinical findings for patients with complex multi-dimensional acute or chronic clinical conditions.

Prescribe medication independently as appropriate, select and order investigations independently, x-ray interpretation for UTC, refer to other professionals independently for tests or examinations with support as required within the scope of agreed clinical guidelines.

Help to Identify the need for improvement and change within the area of responsibility and following agreement and approval, lead on the implementation and development of new policies, procedures and practices and where appropriate with support implement service re-design solutions to better meet the needs of the patients and the service.

Provide clinical leadership, educational and clinical supervision to other staff within the area of responsibility, ensuring that there is provision of the relevant clinical advice, guidance and support to undertake duties.

The trainee advanced clinical practitioner will evidence the advanced clinical practice capability framework across all aspects of the patient pathway to become proficient as an advanced clinical practitioner.

Travel across sites for both clinical and supervisory duties may be required.
