Nimbuscare is looking for experienced and enthusiastic clinicians to join our growing urgent care team. We are a clinically-led organisation committed to innovation, collaboration, and delivering care that puts people first.

Main duties of the job

As part of our team, youll have the opportunity to work across our services, including:

  • Clinical Assessment Service (CAS), based in Holgate, York
  • Weekend Home Visiting Service based in York
  • Face-to-face sessions at York, Selby, Malton, Scarborough and Whitby Hospitals
  • Urgent Treatment Centres at York and Scarborough Hospitals
  • Same Day Urgent Care clinics in York, Scarborough and Whitby

About us

Nimbuscare is a not-for-profit organisation that aims to be a collaborative and agile provider that cares for its people and patients. Nimbuscare is based in York and is owned by all the practices in the city, however we deliver services over a wider footprint.

Nimbuscare deliver Urgent Care services in collaboration with the hospital and GP practices in York, Scarborough, Whitby, Malton and Selby. We also provide community-based frailty care, which focuses on treating people in the community where we can. We also aim to shift services into the community where possible, for example our community diagnostic centres run in partnership with the Acute Trust.

Main Duties and Responsibilities:

Clinical responsibilities:

IUC Hub/Clinical Assessment Service:

Consistently monitor incoming calls, responding promptly based on clinical need

Provide telephone assessment for patients presenting via 111, closing cases over the phone where appropriate

Provide clinical advice for Health Care Professionals and co-ordinate care for patients depending on clinical need

Arrange to review patients face to face at the appropriate UTC/SDUC clinic

Arrange home visits when clinically needed

Provide clinical advice where appropriate to both internal and external staff as required eg triage nurse, GP colleagues, district nurses, ACPs, Paramedics etc

Through prior agreement and where trained to do so, provide Clinical Supervision for GP SpRs/Trainee ACPs whilst on shift

Develop knowledge of referral pathways to maximise the use of community services eg Pharmacy First, the community eye service etc.

Urgent Treatment Centres:

Work within current scope of practice whilst developing and maintaining skills required to work across the full scope of urgent care presentations including minor illness and injury.

Attend further training where needed to become competent in, for example, assessing and managing minor injuries

Provide leadership for minor illness presentations whilst on shift

Work with the Lead Urgent Care Practitioner for minor injuries to ensure appropriate prioritisation of patients based on clinical need

Provide mentoring, supervision and training for GP SpRs and trainee urgent care practitioners whilst on shift.

Provide regular clinical supervision meetings including Designated Medical Prescriber meetings and workplace-based assessments for Urgent Care Practitioners in training.

Ensure clinical coding of patients seen in the UTC is accurate and complete before the end of the shift.

Same Day Urgent Care (SDUC) clinics:

Work autonomously to assess, diagnose and manage minor illness presentations

Through prior agreement and where trained to do so, provide Clinical Supervision for GP SpRs/Trainee ACPs whilst on shift

and consistency.

Operate diagnostic equipment to monitor and assess patients, ensuring accurate readings for informed clinical decisions.

Identify and report faulty equipment, ensuring it is removed from clinical areas for repair to maintain safety.

Completion of event report forms when indicated

Across all aspects of the service:

Appropriately assess, diagnose, treat & discharge or refer patients presenting to the IUC service

Make professional, autonomous decisions in relation to presenting problems and formulate evidence-based management plans

Provide health promotion and preventative health care advice to patients attending the service

Precribe and issue computer-generated electronic prescriptions

Prescribe generically whenever appropriately and in accordance with NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance)

Adhere to all medicines management governance and ensure correct procedures are followed

Clearly and accurately document consultations, including safety netting advice, in a timely manner

Where appropriate, educate patients about how best to use urgent care services

Work collaboratively with other health and social care professionals and services to deliver care to meet patients clinical, social and transport need

Proactively communicate with the On call Manager to escalate any concerns as appropriate to ensure smooth running of the shift

Work with urgent care colleagues working across the service to maximise the effective and efficient delivery of care to patients

Promote the application of standards and codes of Practice set by the Royal College of General Practice, General Medical Council, Department of Health and local NHS trust.

Be aware of service KPIs & work accordingly to assist in achieving compliance

Escalate and report patient safety/experience events or feedback in line with Nimbuscares governance framework

Contribute to and participate in the management of complaints and concerns where appropriate

Provide clinical feedback to enable service development

Understand, maintain knowledge of and adhere to relevant standard operating procedures, policies and pathways

Represent the service and Nimbuscare in a professional manner when in contact with the general public and other professionals

Report any concerns about the safety of the service particularly with regard to any concerns about the behaviour / performance of other staff working for the service.

Inform Nimbuscare of any restrictions which may impact on the post holders ability to undertake urgent care work
