Main duties of the job

As part of our team, youll have the opportunity to work across our services, including:

  • Clinical Assessment Service (CAS), based in Holgate, York
  • Weekend Home Visiting Service based in York
  • Face-to-face sessions at York, Selby, Malton, Scarborough and Whitby Hospitals
  • Urgent Treatment Centres at York and Scarborough Hospitals
  • Same Day Urgent Care clinics in York, Scarborough and Whitby

Job responsibilities

Clinical responsibilities:

  • Autonomously assess, differentially diagnose, plan, implement, and evaluate individual treatment plans, interventions, and care packages for patients, ensuring safe and appropriate management, timely referrals, and safety netting as required.
  • Collaborate with the multidisciplinary team (MDT) to expedite safe and timely triage consultations, interventions, and reviews.
  • Prescribe independently (where legally permitted), review medication for therapeutic effectiveness, and ensure treatment is tailored to patient needs, adhering to contraindications, drug interactions, evidence-based practice, national and practice protocols, and within scope of practice to optimize outcomes.
  • Prioritise and manage personal and team workloads, embedding effective time-management strategies within the team culture.
  • Provide clinical leadership in line with the Patient Safety Incident Response Framework, ensuring clear alignment with company values, strategic direction, and policies impacting care delivery.
  • Maintain effective communication within the MDT and with wider stakeholders, ensuring active participation with senior team members and facilitating clear communication across roles.
  • Communicate independently with external stakeholders and agencies to ensure continuous, high-quality care during both admission and discharge.
  • Support patients and families receiving "bad news," engaging the team as needed to provide appropriate emotional support.
  • Act as an advocate for patients and colleagues, ensuring needs and concerns are effectively represented.

  • While the ACP role is independent, senior support is always available as and when needed.
  • Responsible for assessing ongoing care needs and planning effective transitions, ensuring continuity and quality of care.
  • Ensure clear communication of care plans to the team, fostering collaboration and consistency.
  • Operate diagnostic equipment to monitor and assess patients, ensuring accurate readings for informed clinical decisions.
  • Identify and report faulty equipment, ensuring it is removed from clinical areas for repair to maintain safety.
  • Completion of event report forms when indicated

  • Ensure that medical and patient records are comprehensive, accurate, and up-to-date.
  • Complete relevant coding forms accurately, providing a clear explanation of the patients condition and treatment plan.
  • Take responsibility for maintaining accurate and timely documentation of patient care, including information for referral purposes.
  • Ensure the proper completion of ReSPECT forms and advanced care planning, tailored to the patients level of frailty, clinical condition, co-morbidities and wishes.
  • Accurately document all discussions regarding care, ensuring that conversations with the patient and family members are properly recorded and discussed as appropriate.
  • Report any events via the Nimbuscare reporting system

  • Registered healthcare professional
  • MSc in Advanced Clinical Practice or digital badge from a supported advanced practice portfolio, or equivalent.
  • Completed a portfolio of professional development that demonstrates an advanced level of clinical practice in line with the HEE (2017) framework for ACP.
  • Independent non-medical prescribing.
  • Basic Life Support (BLS).
  • Clinical Skills Level 7.
  • Formal research module completed at Level 7 (Masters level) or equivalent, e.g., supported portfolio and digital badge.
  • Will work towards and complete any relevant credentialing routes available and applicable to the clinical area.
  • Minor Illness qualification or equivalent.
  • Paediatric clinical skills or equivalent

  • ALS Provider/Instructor
