Duties and Responsibilities:
The duties and responsibilities to be
undertaken may include any or all of the items in the following list. Duties may be varied from time to time under
the direction of the Medical Director or Board of Directors, dependent on
current and evolving workload and staffing levels.
Clinical Responsibilities:
You
will undertake a variety of duties, mostly offering support and advice to
System providers regarding vulnerable patients in Community as part of an
MDT, undertake best interest discussions with patients and family members
and help co-ordinate a response to support people in need to stay home
whenever possible.
You may
be needed to visit a complex frail patient at home or undertake telephone
consultations.
Make
professional, autonomous decisions in relation to presenting problems.
Assessing
the health care needs of complex patients with undifferentiated and
undiagnosed problems, highlighting which services would support them best to
stay in their home setting and independent whenever possible.
Cover
the clinical management of patients as appropriate.
Record
clear and contemporaneous computerised consultation notes to agreed
standards.
Compile
and issue computer-generated acute and repeat prescriptions.
Prescribe
generically whenever appropriately and in accordance with NICE guidance
and Good Prescribing Practice (as defined by British National Formulary
guidance).
Work
with and support other members of the MDT.
Promote
the application of standards and codes of Practice set by the Royal
College of General Practice, General Medical Council, Department of Health,
British Geriatrics Society and local NHS trust.
Participate
in national frailty training and maintain high volume of CPD in Frailty
related conditions with the aim of obtaining the Diploma in Geriatric Medicine.
Other Responsibilities:
Involve and contribute to the
mentoring, supervision and training of the broad multi-professional
clinical team.
Be aware of and comply with all
relevant policies/guidelines, e.g. prescribing, confidentiality, data protection,
health and safety.
Commitment and contribution to
achievement of the highest possible quality standards.
Commitment to life-long learning
and audit to ensure evidence-based best practice.
Contribute to evaluation/audit and
clinical standard setting within the organisation.
Attendance at regular in-house
governance and educational meetings and training as well as events at
other agencies as appropriate.
Assist with the training of other
staff members.
Contribute to the summarising of
patient records and Read-Coding patient data.
Take
responsibility that any equipment either clinical or otherwise, used when
working, is taken due care of and locked away at the end of every working
session.

and support the introduction of new working processes to optimise quality.
Communicate
to ensure that all employees operate as an effective team.
Build and
maintain relationships with other healthcare professionals, including GPs,
GP surgery personnel, care home personnel and CCG personnel etc.

Be fully aware of and understand your duties

and responsibilities arising from the DOH Policy No Secrets (2000), and

Ensure the Nimbuscare Safeguarding Lead is
alerted to, and kept fully informed of, any concerns you may have in
