Job summary
We are seeking to appoint a number of GPs with an interest in Frailty to support the delivery of the various developing Frailty Services.
You will need to hold the relevant professional registration (GMC) and have experience of Triage / Telephone Assessment. Experience of working in Geriatric Medicine is desirable. Any successful candidates would join our growing team of GPwERs in Frailty and be supported to obtain their diploma in Geriatric medicine as well as join our regular education sessions.
Over 85s are set to increase in York by 53% by 2033 and we need to anticipate the needs of a growing number of frail patients. In its first 6 weeks the Frailty Hub has avoided a total of 100 unnecessary 999 calls by supporting providers to make decisions regarding frail patients in crisis and working with teams to come up with supportive intervention plans. Follow up of their first 60 patient contacts shows that these patients are staying home and well up to 4 weeks following intervention. This model is growing very quickly and is gathering national interest and we need a growing team of GPs with an interest in Frailty to support us to deliver the array of Frailty Services that Nimbuscare are helping to develop.
This is predominantly a face-to-face role based at Acomb Garth Community Centre between the hours of 8.30am and 5pm but if doing any evening shifts between 5 and 8pm then this can be done remotely from home.
As the clinician you will be working on System One.
Main duties of the job
- Support and advise System providers regarding vulnerable patients in Community as part of an MDT, undertake best interest discussions with patients/family members and help co-ordinate a response to support people in need to stay home.
- Visit complex frail patients at home or undertake telephone consultations.
- Make professional, autonomous decisions in relation to presenting problems.
- Assessing the health care needs of complex patients, highlighting which services would support them best to stay in their home setting.
- Cover the clinical management of patients
- Record clear and contemporaneous computerised consultation notes to agreed standards.
- Compile and issue computer-generated acute and repeat prescriptions.
- Prescribe generically whenever appropriately and in accordance with NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance).
- Support other members of the MDT, including but not limited to: social workers, therapists, social prescribers, voluntary support workers, frailty nurses, dieticians, UCR paramedics and other Frailty GPs.
- Promote the application of standards and codes of Practice set by the Royal College of General Practice, General Medical Council, Department of Health, British Geriatrics Society and local NHS trust.
- Participate in national frailty training and maintain high volume of CPD in Frailty related conditions with the aim of obtaining the Diploma in Geriatric Medicine.
