Job summary

Nimbuscare are looking to recruit ACP’s, ANP’s, UCP’s and ECP’s to work alongside the York Community Response Team to triage and facilitate urgent community response visit requests.

Main duties of the job

The requirements of the role will be to do medical UCR visits and assess patient needs at home, and step-up, prescribe, admit, arrange CRT/carer support to enable the patient to stay at home as appropriate.

The role will also involve supporting the frailty virtual ward service.

Remote senior clinical support will be in place as needed.
