We have an exciting opportunity for a Palliative/End of Life Frailty Lead to join our team at St Lukes Hospice, working predominantly in the community setting.

You will use your experience and specialist palliative care knowledge to work closely with health professionals to deliver appropriate management, advance care planning, undertake assessment and medication review for end-of-life care.

As part of our commitment to a continuous improvement strategy for this group of individuals and, in order to reduce variation in care with the focus being on standardising practice we have created this innovative post.
