Job summary

The South Tees area comprises of 46 general practices with a long history of working together in their respective localities of Langbaurgh, Eston and Middlesbrough. It covers a total population of 290,000 and includes some of the most deprived areas in the country. The area has traditionally been one of the highest levels of emergency and unplanned attendance and hospital admissions in the country, currently ranked 4th highest

Main duties of the job

To practice autonomously as a lone practitioner without direct supervision as an Advanced Paramedic Practitioner/Advanced Nurse Practitioner across the South Tees area. The role will involve the triage, assessment, diagnosis, treatment or referral of patients presenting with undifferentiated and undiagnosed conditions. The post holder will work autonomously within the Advanced/Emergency Care Practitioner’s level of competency.

The role will also involve elements of proactive support and education to staff and managers within care homes and to care home forums.

Referrals and requests may come from various sources including:

An exciting opportunity has arisen to join ELM Alliance Ltd. We are a GP federation that promotes a high level of care and respect within the team, delivering the Care home Enhanced Rapid response service (CHERRS). Part time hours available (10 hours per week over one shift) on a permanent basis with access to NHS pension scheme.

The ELM Alliance is a not-for-profit Federation of all independent GP practices throughout South Tees which covers over 300,000 patients. Members of the Alliance remain independent organisations, whilst collaborating in the development of local primary care.

The ELM Alliance CHERRS service was first commissioned in June 2017 and has been successful in securing ongoing funding from 2021. The service is provided 7 days a week, 8am – 6pm including bank holidays

The aim of the service is to target urgent, emergency, unscheduled and acute episodes of care in patients residing in care homes who would otherwise be at risk of hospital attendance or admission. The service provides a rapid response which ensures the unwell patient receives prompt examination and treatment by an advanced practitioner You will be expected to assess episodes of acute/chronic illness and will be competent in the clinical examination, assessment, diagnosis and treatment of the patient, including prescribing where appropriate

Requirements

To succeed, you will be a practicing advanced nurse practitioner/advanced paramedic with previous experience in urgent and emergency care. You will demonstrate a high standard of clinical skills. You will also have the ability to communicate, liaise and interact effectively with multi professional teams across organisational boundaries, including the ambulance service, primary care, social services and secondary care providers.

Strong organisation and time management skills are essential, as is the ability to prioritise your workload. You will also have experience using various clinical IT systems, in addition to basic IT literacy.

  • University accredited advanced physical assessment skills Course level 7/ ECP qualification.
  • Non-medical prescribing qualification.
  • Previous experience in urgent or emergency care.
  • Ability to manage and prioritise workload.
  • Proven ability to work autonomously and as part of a team.
  • Current knowledge of government targets, initiatives N.I.C.E. guidelines and their impact on clinical practice.
  • Knowledge of adult protection issues.
  • Understanding of clinical governance and its significance and impact upon practice.
