Job summary

An exciting opportunity has arisen to join the management team of the Total Health Excellence East and West PCNs as our Project and Transformation Manager.

The ideal candidate will need to be proactive, have a passion for primary care, and have a proven track record in delivering improvements, especially digital transformation.

The remit of this role is broad and evolving, with opportunity for the post holder to help shape the post and PCN transformation priorities. The responsibilities will vary according to the needs of the PCN but will include leading a wide range of transformation and digital projects, achievement of primary care improvement targets and initiatives including IIF/QoF, and supporting the education and training agenda.

The role requires flexibility, initiative taking, relationship building skills and attention to detail in delivering high-quality day-to-day operations. The post holder will be required to deliver small to medium change initiatives and projects, both across PCN Practices with partner agencies, in order to deliver the changes required in all geographical areas of THE PCNs.

Significant experience and expertise in a range of primary care electronic clinical and managerial systems is essential, including EMIS. Excellent project management, analytical and digital skills are also required.

We operate across the Dover and Folkestone areas and eight GP practices form our PCNs. Our PCNs are led by a Clinical Director, who is a GP, supported by a team of around 50 highly qualified health professionals.

The PCN Executive Team together with the Head of Strategy & Planning will identify the work programme and outline objectives for the post holder. In most cases, an Executive Director will be accountable for each project and the post holder will report to this Executive Director (SRO) and Head of Strategy & Planning, the Senior Responsible Owner (SRO) for the project and PCN Executive as appropriate, for delivery of the relevant project.

Job responsibilities

To lead in the development, planning, implementation, management, monitoring and evaluation of specific small to medium sized projects or change initiatives, as part of the wider programme of change for THE PCN working closely with key internal and external stakeholders; supported where appropriate.

To lead and implement a strategic plan that includes a robust digital infrastructure, including supporting PCN and practice staff to use new digital systems.

Coordinate training and development of PCN staff.

Achievement and reporting of primary care improvement targets and initiatives, including IIF/QoF.

To provide oversight and accountability to the delivery of PCN contracts and specifications

To support the implementation of objectives and priorities for network delivery in line with operational capacity, financial parameters and contract requirements

Promote the engagement of all PCN practices and act as an ambassador for the network.

To proactively build relationships with system partners to help influence proactive and new models of care

To use business intelligence tools to aid data-driven decisions

Identify and record risks and issues and develop contingency plans with Network Leads

Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented

To drive issues through to resolution, acting as a point of expertise

To implement network-based projects

To support the collation of financial information

To monitor and check the accuracy of the submissions for the DES and contractual requirements

Compiling and presenting a range of performance and financial reports for the PCN and regional/national authorities
