Job summary
As an Advanced
Nurse/Clinical Practitioner you will be
confident working autonomously carrying out clinical consultations, providing
assessment, diagnosis, treatment and medication through independent prescribing.
In addition, your experience will provide management and leadership to the
teams delivering urgent treatment and improved access services. To succeed in
this role you will need excellent interpersonal, communication and
organisational skills.

Main duties of the job
We are looking for an Advanced Clinical Practitioner who has experience working in urgent or primary care. The post holder will spend 20% of their time providing management support to the staff and 80% of their time in clinical practice. This will give you an opportunity to develop your management and leadership skills.
Undertakes
Clinical Practice at an advanced level using expert knowledge and clinical
skills to deliver holistic care to people accessing primary health care
services.
Assesses
diagnoses and treats patients with a range sub-acute and chronic medical
conditions.
Refers
patients to other medical specialities for assessment if deemed appropriate.
Refers
patients as appropriate to other members of the multidisciplinary team,
secondary care and to other statutory and voluntary organisations including
referral for x-ray, ultrasounds etc.
Works
within professional guidelines and codes of conduct.
Works
within the Scope of Practice and their Scope of Prescribing Practice
Works
within all relevant practice policies and procedural guidelines e.g., infection
control, chaperoning, risk management.
Independent
Prescriber.
Works with and
supports the UTC and EA Lead in the management of staff and running of the
services, acting as a deputy when the Lead is not available.

Channel Health Alliance (CHA) is a collaborative federation of 28 GP practices in and around the South Kent Coast that are within NHS Kent & Medway. To enable primary care to provide safe, quality services to residents, the local GPs decided to set up a Federation that would enable them to deliver services at scale.
The CHA team deliver a variety of directly contracted or PCN hosted services including UTC and Improved Access services Home Visiting, Enhanced Health in Care Homes as well as services developed in response to the Coronavirus Pandemic including vaccine centres in Dover & Folkestone.
