Objectives
of Devon Personalised Care Programme
The programme
objectives are to maintain and enhance the social prescribing programme in
Devon, founded on principles of quality improvement and workforce
empowerment.
The focus for Devon
is to
o Support good rollout of the personalised care agenda through training
and education
o Support introduction of children and young people social prescribing
services
o Continuous improvement of existing adult social prescribing services
o Work with local communities
o Ensure we live our values incorporating patient voices and lived
experiences into our work
The Role
Applicants can
be drawn from any suitable primary care background. The Fellow will focus on
the sustained strengthening and embedding of the Personalised Care Agenda in
primary care and PCNs across Devon, working very closely with the Devon ICS
Personalised Care, Training Hub Teams, Clinical and Managerial Leads and PCN
Clinical Directors.
The areas of
focus for the Fellow are:
o Contribute to successful delivery of the PCN DES for Personalised
Care
o Strengthened collaborative working
between Devon Personalised Care, Training Hub and PCNs
o Quality engagement with PCNs to
increase the knowledge, skills, and confidence in implementing the Universal
Care Model in primary care
o Development of personalised care strand of the Devon Training Hub
Induction programme
o Work collaboratively to support
embedding of personalised care roles Care Coordinators, Health &
Wellbeing Coaches and Social Prescribing Link Workers in PCNs including
development of peer support models and appropriate training programmes
The Fellow
will:
o Engage with PCNs and Clinical
Directors to increase the knowledge, skills, and confidence in implementing the
Universal Care Model in primary care
o Support promotion of Universal Care
Model including personal health budgets and shared decision making amongst
clinicians
o Support the development of Devon Training
Hub induction programme for personalised care roles
o Develop peer support models and sustainable ongoing training
programme

Responsibilities Main Duties
Working with the Devon ICS
Personalised Care and Devon Training Hub teams to advance Devon ICS and Devon
Training Hub aims and objectives relating to the Personalised Care Agenda and
roles.
Monitor policy developments relevant to all aspects of Personalised
Care Agenda
Supporting communities of practice
With the Devon ICS Personalised Care
and Devon Training Hub Programme Managers develop ideas, analysis and projects
relating to innovation and improvement in Personalised Care
Contribute to the development and
planning of projects
Engage with PCNs and Clinical
Directors to increase the knowledge, skills, and confidence in implementing personalised
care in primary care
Support the development of Devon
induction programme for ARRS roles for the socialised care roles
Support communities of practice for
personalised care in collaboration with PCNs
Develop an ongoing training programme
package across Devon
Corporate
Responsibilities
Support the Devon ICS Training Hub and
Devon ICS Personalised Care Team to deliver high quality projects and workforce
This will be achieved through
effectively fulfilling the accountabilities and responsibilities of the role
detailed above.
Demonstrate sound judgement in the
absence of clear guidelines or precedent, seeking advice as necessary from
peers or the clinical lead as appropriate on any projects or work within the
postholders remit.

The post holders will pursue a
programme of CPD in accordance with any relevant professional registration or
statutory requirements, whilst maintaining appropriate awareness of service
provider requirements.

The post holders must, if required to
