Job summary

Devon Training Hub are looking to recruit a Personalised Care Fellow to support General Practice in developing and embedding Personalised Care roles in PCNs and Practices. The main focus of the role will be:

Contribute to successful delivery of the PCN DES for Personalised Care

Strengthened collaborative working between Devon Personalised Care, Training Hub and PCNs

Quality engagement with PCNs to increase the knowledge, skills, and confidence in implementing the Universal Care Model in Primary Care

Development of personalised care strand of the Devon Training Hub Induction programme

Work collaboratively to support embedding of personalised care roles Care Coordinators, Health & Wellbeing Coaches and Social Prescribing Link Workers in PCNs including development of peer support models and appropriate training programmes

The role will be based with Devon Training Hub at our Plymouth office but will require travel to various locations throughout Devon to training locations. It is expected that this role will be fixed term for 12 months, at 8 hours per week to fit around your General Practice role.

Accountable to: Appointed fellows would be hosted by and within the Devon Training Hub but would be accountable to Devon ICS as the funder. This will include attending network meetings and working with relevant Devon ICS & WT&E colleagues as part of projects relating to their fellowship.

o Support the development of Devon Training Hub induction programme for personalised care roles

o Develop peer support models and sustainable ongoing training programme

We are a small and friendly team based in Plymouth but with team members based in Devon, Cornwall & Somerset and we offer the ability to work flexibly at the role requires.

o Continuous improvement of existing adult social prescribing services

o Work with local communities

o Ensure we live our values incorporating patient voices and lived experiences into our work

The Role

Applicants can be drawn from any suitable primary care background. The Fellow will focus on the sustained strengthening and embedding of the Personalised Care Agenda in primary care and PCNs across Devon, working very closely with the Devon ICS Personalised Care, Training Hub Teams, Clinical and Managerial Leads and PCN Clinical Directors.

The areas of focus for the Fellow are:

o Contribute to successful delivery of the PCN DES for Personalised Care

Monitor policy developments relevant to all aspects of Personalised Care Agenda

Supporting communities of practice

With the Devon ICS Personalised Care and Devon Training Hub Programme Managers develop ideas, analysis and projects relating to innovation and improvement in Personalised Care

Contribute to the development and planning of projects
