Local Care Direct is a non-profit Social Enterprise operating across 24 hours a day, 7 days a week, 365 days a year, to provide various primary and urgent health care services across West Yorkshire.
We have a Fixed-Term vacancy available for a Full-Time Programme Lead within the Urgent Community Response (UCR) Provider Alliance.
The Programme Lead will work for the Kirklees Urgent Community Response (UCR) Provider Alliance and operate on behalf of all partners in Kirklees to deliver the requirements of the Urgent Community Accelerator programme and service.
The Urgent Community Response service is designed to provide a rapid intervention delivered within 2 hours by a team of skilled professionals, to provide the support residents need to remain independent and avoid an admission to hospital.
This vacancy is Fixed Term for 12 months.
Salary: Within the region of £47,126 to £53,219 per annum (Band 8A Equivalent)
Location: Will be worked across Kirklees with a base of Local Care Direct, Unit 14, Longbow Close, Bradley, Huddersfield, HD2 1GQ
Hours: 37.5 hours per week full-time office hours (flexibility required)
Main duties of the job
The Programme Lead will lead the delivery of the UCR Programme for the Kirklees Health & Social Care system. They will work in partnership
with a network of colleagues from the UCR Provider Alliance to ensure that the
approach to delivering the UCR programme is clearly articulated and that each
partner is supported in the delivery of the key objectives.
They will work closely with leaders in the Kirklees system, & liaise
with the West Yorkshire & Harrogate Integrated Care System (ICS) & NHS
England regional and national teams, providing high quality support and leadership
in relation to the programme.

- updating UCR demand/capacity modelling and review
- Continued Stakeholder Engagement and communication
- Develop a risk sharing agreement between the UCR Providers Alliance members
- Support completion of contract waiver and direct award contracts
- Identify opportunities for service development and clinical model
- Review overhead costs within UCR Alliance Providers and work with Kirklees place on a systematic and consistent approach.
- Continue to work with referral sources to drive up demand for the service to maximise the usage of available capacity, particularly from 111 and 999
- Further integrate and collaborate across community and Ageing Well services to develop efficiencies in 2022/23 onwards i.e. Enhanced Health in Care Homes, Discharge to Assess, other home visiting offers, intravenous therapy.
- To streamline and incorporate UCR into the collaborative work being undertaken by Kirklees Providers on Kirklees and West Yorkshire on urgent and unplanned community services i.e., Links to Clinical Advisory services, virtual wards, merger of unplanned/rapid care teams across Primary and Community care

- Liaise with the West Yorkshire ICS to ensure the profile of Kirklees is raised and ensure the WY Ageing Well Lead supports the links and feedback with national leads in NHS England and in other UCR Accelerators, ensuring that they are kept informed of progress and achievements in Kirklees.
- Operate effectively at both local and ICS level, to ensure a coherent approach, and to maximise the opportunities to use to funding flexibly to support existing priorities. In particular, ensure that there is a direct link between this programme and the nine Primary Care Networks in Kirklees.
- Align the design, development and delivery of key services with the strategic priorities of the Health and Wellbeing Board, and work with both the Kirklees Integrated Board to ensure system ownership of the programme.
