This is an exciting time in for working in Primary care Services and we are looking for a dynamic, enthusiastic, flexible, and highly organised PCN Project Manager to support the development of our Primary Care Networks covering sixteen practices in Grantham and Sleaford.

Main duties of the job

The PCN Project Manager will work as part of the PCN Teams, leading the delivery of a range of projects and will be responsible for ensuring the smooth transformation and implementation of new services. To achieve this, you will be required to engage with key internal and external stakeholders, be able to identify risks and opportunity and collaborate with relevant colleagues to resolve issues as they rise.

Along with focus on innovation and change, the role will include supporting GP practices to deliver the PCN DES contract and some responsibility for management of staff and operational services.

Working as a key member of the K2 Senior Operations team, this is a fantastic opportunity for some one that really wants to have an impact, to join a supportive and committed team.

The Better Lives Lincolnshire Integrated Care System sees us working in collaboration with the Primary Care Network Alliance, Secondary Care, Community Health, Mental Health, Social Care as well as Local Authorities, and the Community and Voluntary Sector.

2. Purpose

You will work with the K2 Operations Team and partners across the system to design and deliver innovative new health care services and to manage and develop existing K2 operational services and deliver the PCN contract requirements.

With a portfolio of potential initiatives already within the business pipeline, the Project Manager will support service design and operational modelling, the production of business cases and the implementation and embedding of change plans.

The role holder will ensure that solutions are developed and presented with clear and comprehensive project plans and will be responsible for engaging with stakeholders to implement these plans.

New services will meet the needs of commissioners in key areas of patient care, value for money and compliance and deliver sustainable business in line with the K2 business plan.
