Job summary

Enhanced Access model of care will see K2 Healthcare PCN offer routine general practice appointments available at patients own GP practice, or another local NHS location, on weekday evenings from 6.30pm 8.00pm and on Saturdays from 9.00am 5.00pm.

Operated by a multidisciplinary team, including GPs, nurses, and other healthcare professionals delivering various appointments depending on clinical need, including face to face and remote appointments via telephone or online.

This is a consistent national access offer to provide routine appointments at evenings and weekends to meet locally determined demand, alongside effective access to out of hours and urgent care services.

Additionally, this provides a solution for patients who may otherwise attend A&E.

Overall, this will enhance primary care access across Lincolnshire while also increasing the level of engagement and participation of all practices working together and closer to other professionals from across the health and social care system.

Main duties of the job

The post holder will work as an autonomous practitioner, responsible for the provision of medical services to Enhanced Access (EA), delivering an excellent standard of clinical care whilst complying with the EA contract.

Furthermore, they will adhere to the GMC standards for good medical practice, contributing to the effective management of the service, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team.

We are a GP Federation covering 16 practices. 10 in Grantham and surrounding villages and 6 in Sleaford. We cover 130,000+ patients across all sites.

We are continuing to support our practices resilience through the Additional Roles Reimbursement Scheme (ARRS); and supporting and developing neighbourhood working across our population.

We have teams of Clinical Pharmacists, Pharmacy Technicians, Social Prescribers and Link Workers, Occupational Therapists, Care Coordinators and Paramedics.
