Job summary

The Home-Visiting Advanced Nurse Practitioner/Enhanced Practice Nurse role exists to support the delivery of the Federation Home-Visiting service working alongside the Paramedic Practitioners, and supported by the Service Delivery Manager, Federation HR and Admin Officers, Directors and Clinical Directors.

The Advanced Nurse Practitioner/Advanced Clinical Practitioner is part of a clinical team providing home visits to patients registered with one of our sixteen practices. The ANP/EPN is a professionally autonomous practitioner managing undiagnosed problems using advanced nursing/clinical skills. The post holder will assist in developing protocols and guidelines within the service and work with clinical colleagues in ensuring the highest standards of care for all patients.

The home visiting team is an integral part of the neighbourhood working model that is embedded within South West Lincolnshire. While the team responds to acute patient needs, taking an inquisitive and holistic view is just as important to feedback and engage with GP Practices and Care Coordinators to plan for ongoing support needs. The patient cohort will predominantly be frail and complex, with co-morbidities and housebound in their own homes or living in care homes.

Main duties of the job

An Advanced Nurse/Clinical Practitioner (ANP/ACP) delivers high-quality, autonomous care across clinical, professional, and managerial domains. Core duties include conducting home visits using advanced assessment, diagnostic, and prescribing skills to manage acute and chronic conditions. The role involves arranging and interpreting investigations, developing care plans, and promoting health and well-being. Practitioners prioritise health needs, manage mental and community health presentations, and liaise with external agencies to support vulnerable patients. Safeguarding is a key responsibility, alongside accurate clinical coding and documentation. Professionally, the ANP/ACP adheres to NMC standards, maintains confidentiality, and engages in continuous learning. Prescribing is carried out in line with legislation and local guidelines. Proficiency in clinical systems like SystmOne and WebV supports safe, effective care. The role also includes managing workload, collaborating with colleagues, and contributing to service development. Participation in meetings, audits, and quality improvement ensures alignment with best practice and patient-centred care.

Clinical Responsibilities

Participate in the administrative and professional responsibilities of the practice team.

Accurate and legible notes of all consultations and treatments are recorded in the patients notes.

Ensure clinical systems kept up to date, recording and/or amending accurate details.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Attend and participate in practice/PCN and multi-disciplinary meetings as required.
