Job summary

Our Stoke Community Drug and Alcohol service are looking for a Hospital Link Worker to join our team. If you are enthusiastic, creative and passionate about helping others, we'd love to hear from you.

Main duties of the job

The Hospital Link Worker will provide support for people in hospital who are using drugs or alcohol who want to engage in treatment. You will form links with the hospital to ensure continuity of care, establishing & maintaining a quality hospital liaison service developing inpatient & outpatient services for those with substance related issues. You will liaise between:

  • Hospital and, community pharmacies
  • Hospital safeguarding teams inpatient and community Mental Health Teams
  • Accident & Emergency to provide early interventions for those with drug or alcohol related health concerns
  • core WithYou teams including our Clinical Lead

This is an integral role for With You - as a frontline worker, you will deliver continuity of care through effective partnership working, care coordination, delivery of interventions and key work responsibilities. This is a full time, fixed term position working 37.5 hours per week until 31/03/2026. Some Saturday working is required on a rota basis and the salary is £23,821 to £26,500 per annum.
