Job summary

We have an exciting opportunity in our Cornwall services for an Hospital Outreach Worker to join our busy and dynamic team based in Truro. This is an exciting and challenging role that works from a trauma informed perspective based in the community covering all of Cornwall. Due to the outreach basis of this role, a full drivers licence and access to a vehicle is required.

Main duties of the job

The Drug and Alcohol Outreach Service works alongside the Adult Community Service and the Young People's Service to ensure that an accessible treatment offer is available to those who find themselves rough sleeping or in insecure accommodation. The Drug and Alcohol Outreach Service is countywide and promotes an holistic multi-agency response to the needs of the individual. The Hospital Outreach Service provides a rapid response to individuals who are frequent attenders at the Royal Cornwall Hospital in Truro and have issues related to substance use. The team works within a multi-disciplinary model of practice with partners in safeguarding, housing and homelessness services, domestic abuse services, as well as primary and secondary health care and mental health providers. We welcome applications for this Hospital Outreach Worker role from candidates with experience in the following areas: Outreach, Mental Health, Substance Misuse, Drugs and Alcohol and other complex needs.
