This role is responsible for
Providing operational support to PCNs or practices to improve processes and outcome targets in relation to Long Term health conditions (with an initial focus on diabetes)
Line management of a team of care coordinators
Supporting
the longer term implementation of Integrated models of care for long term
health conditions

Main duties of the job
Key duties you will be
responsible for:
Facilitating engagement with
practices/PCNs regarding the achievement against the NDPP milestones,
increasing referrals and attendance for Structured Education and the Low
Calorie Diet, understanding the weight management services available, delivery
against the 9 care processes and Diabetes Treatment Targets as indicated by the
Eclipse Diabetes Module
Supporting practices/PCNS to
address variation for NDPP, Structured Education, Low Calorie Diet, other
Weight Management initiatives, 9 care processes and NICE Treatment Targets
through training, education and data analysis
Developing, with support, the
services and/or information required for key practice and commercial
requirements such as further data collection or new EMIS searches
Providing expert advice related
to the development of the Eclipse Diabetes Module to enable practices to
routinely use the system and address variation across all PCNs
Support development of each PCN
delivery model for Diabetes and MDT working with secondary and community care
Supporting the longer term implementation
of Integrated models of care for long term health conditions
Reporting and ensuring the One
Care and ICS executive team is kept up to date on progress and risk management

One Care is the GP federation that
represents and supports practices in Bristol, North Somerset and South
Gloucestershire (BNSSG). Established in 2014, we have 77 member practices
across the area who provide healthcare for around 1 million patients. Our
vision is to enable general practice to survive and thrive. We are a Community
Interest Company, focused on delivering benefit for our practices.
We offer direct support to practices with
many different aspects of running their day-to-day businesses through our
Practice Support Team. We also support practices to work at scale at PCN,
locality and BNSSG-level, holding contracts on their behalf as required and
offering relevant expertise in areas such as digital, analytics and
communications. We work closely with our practices to amplify their voices in
discussions and decisions about the delivery of healthcare in our area.
In September 2020, we established a GP
Collaborative Board (GPCB) to accelerate and amplify the representation of
general practice in the wider health and social care system. The GP
Collaborative Board will enable general practice to engage effectively with the
developing Integrated Care System and to support development of strong place-
based Integrated Care Partnerships.
This role is aligned to the Integrated
Care System (ICS) Diabetes Programme Board objectives and specific aims of the
sub-groups related to the National Diabetes Prevention Programme, 9 care
processes and NICE Treatment Targets

To work with general
practice to achieve the aims of recovery plans for NDPP, Structured
Education, 9 care processes and NICE Treatment Targets whilst creating a
sustainable approach to delivery
To work with the OC
teams to develop effective evaluation methods for this work particularly NDPP
data collection and development of the Eclipse Diabetes Module
To work with the
Communications and Engagement Team to develop effective communication and
training materials related to the relevant areas
To ensure early
identification and management of any risks, providing regular and clear
reports to Senior Management
To contribute
information to OCs reporting methods human, technical and financial
To report progress
and exceptions on a regular basis
To manage the PCN
engagement activities, communication with the One Care team and other key
stakeholders including the related ICS sub-groups related to NDPP, Treatment
Targets and Low Calorie Diet
To attend meetings
on behalf of OC, as requested, and to be an advocate for the areas and its
