Job summary

An exciting opportunity has arisen to come and join our dynamic Wellbeing Team supporting families and carers. The Family and Carer Support Co-ordinator will work with family members and carers, to help them identify their needs, offer support and, where appropriate, signpost them to appropriate services within Longfield and beyond.

Main duties of the job

We are looking for an enthusiastic, innovative, organised person with excellent communication skills to take the service forward and someone who is passionate about supporting those that care for someone with a life limiting illness. If you have recent experience of working with and supporting carers/ families and an insight into the demands placed on the carers and family of people affected by life limiting illness, then we would love to hear from you. The successful candidate will be able to demonstrate excellent communication skills alongside a commitment to developing a positive team culture focused on person centred care.

About us

Longfield is a community hospice charity, in a beautiful setting near Stroud. We believe that everyone touched by a life-limiting illness in Gloucestershire should benefit from free hospice care. We support people to live well after diagnosis through a range of Wellbeing Services for them and their loved ones and, when the time comes, we provide Hospice at Home care, enabling them to have the choice of dying in the comfort of their own home. We also provide support to their families and carers, including counselling and bereavement counselling.

Essential

  • Recent experience of working with and supporting carers and family.
  • Insight into the demands placed on the carers and family of people affected by life limiting illness.
  • Previous experience of working in a general health and social care environment, i.e. hospital, social services or care role.
  • Knowledge and experience of assessing peoples needs (face-to-face, online and by telephone).

Desirable

  • Experience of working within a multi-disciplinary team.
  • Experience of working with volunteers.
  • Experience of working with patients and families prior to and following a death.
  • Experience of diverse communities and understanding the influence culture and ethnicity has on responses to loss and change.
  • Experience of being involved in projects to improve and develop services.
  • Experience of working and liaising with statutory and voluntary organisations.
  • Experience of working in palliative/end of life care setting.
  • Experience of using electronic patient record systems.

Qualifications

  • 5 GCSEs A - C including English & Maths.
  • Proficient IT skills, including in the use of Microsoft Office applications.
  • Effective communication and interpersonal skills.
  • Innovative and resourceful.
  • Able and willing to seek support and advice when appropriate.
  • Ability to manage and prioritise own time and workload.
  • Solution focussed, able to analyse and problem solve.

  • Level 3 qualification, diploma or certificate in health and social care
