Job summary

Charnwood GP Network is a Federation of 21 General Practices covering the localities of North and South Charnwood in the area in and around West Leicestershire. The network encompasses 4 Primary Care Networks (PCNs) which work together collaboratively as part of the federation.

This exciting opportunity will involve working closely with the Head of PCN Development, PCN Clinical Directors and the Integrated Neighbourhood Team (INT) Chair, along with the PCN GP Practices to deliver and develop new clinical services and pioneer new ways of collaborative working for the benefit of our Patients.

Main duties of the job

To operate with the Head of PCN Development & PCN CDs to support the delivery of PCN initiatives and projects, acting as the liaison between the Federation and Practices.

Work with the Head of PCN Development to utilise GP Practice clinical systems (SystmOne or EMIS) and population health data from a variety of sources, to proactively identify relevant cohorts of patients to support practices to deliver personalised care and meet the outcomes of the PCN DES / IIF Service Specifications,

Work proactively with other PCN Additional Roles staff to identify patient service needs or inequalities and work closely with the Federation team and PCN CD to work up any projects or initiatives to support the delivery of patient care.

Work with practices and other Care Coordinators to re-design Enhanced Health in Care Homes

Provide admin support for the Enhanced Access Service

Plan appointments according to population needs in Enhanced Access appointments

Support the PCN in improving overall patient care

To support the Head of PCN Development in administrative duties

To support the PCN Clinical Directors in collecting information from member practices

Gather information and undertake enquires for the PCN and the broader group as and when necessary.

Build a robust knowledge of the PCN and member practices.

About us

Charnwood GP Network (CGPN/ Federation) is the employing organisation for the Additional Roles required to enable their 21 Practices to fulfil the service requirements of the PCN DES.

There are 4 PCNs, Beacon, Carillon, Soar Valley and Watermead, within which are 21 Practices.

The Federation Offices are sited within Loughborough, where the Federation Team are based, and all our ARRS teams are deployed to work within their PCNs / Practices.

Administrative Responsibilities:

To operate with the Head of PCN Development & PCN CDs to support the delivery of PCN initiatives and projects, acting as the liaison between the Federation and Practices.

Work with the Head of PCN Development to utilise GP Practice clinical systems (SystmOne or EMIS) and population health data from a variety of sources, to proactively identify relevant cohorts of patients to support practices to deliver personalised care and meet the outcomes of the PCN DES / IIF Service Specifications, e.g., Enhanced Access, Early Cancer Diagnosis, Cardiovascular Disease Prevention and Diagnosis and Enhanced Health in Care Homes.

Work proactively with other PCN Additional Roles staff to identify patient service needs or inequalities and work closely with the Federation team and PCN CD to work up any projects or initiatives to support the delivery of patient care.

Work with practices and other Care Coordinators to re-design Enhanced Health in Care Homes

Provide admin support for the Enhanced Access Service (working at scale across PCNs), working with the Lantum eRostering tool to advertise and accept shifts, ensuring that clinical rotas are applied to the clinical system accordingly and provide support for altering clinics if there is staff leave or sickness.

Plan appointments according to population needs in Enhanced Access appointments, including rota management/ arranging premises and consumables required, working alongside the administrative teams within all linked GP Practices.

Support the PCN in improving overall patient care through promotion of services available to them locally within the PCN and the wider health system.

To support the Head of PCN Development in administrative duties relating to other PCN activities and work streams, these will change and evolve each year.

To support the PCN Clinical Directors in collecting information from member practices, identifying anomalies, and summarising such findings, this could also include presentation fo such facts within internal and external meetings.

Gather information and undertake enquires for the PCN and the broader group as and when necessary.

Build a robust knowledge of the PCN and member practices.

Patient Focused Duties:

Help people to manage their needs through answering queries, making and managing appointments and ensuring that people have written or verbal information to help them make choices about their care

Liaise directly with care homes and other key providers, and compile and circulate relevant information across stakeholder groups

Communicate effectively and sensitively using language appropriate to the patient and their carer and their level of understanding, using a Making Every Contact Count approach.

Provide accurate, impartial information, support and guidance to patients and their carers to enable them to make choices about their care.

Raise awareness of shared decision making and decision support tools and assist patients to be more prepared for shared decision-making conversations.

Other Responsibilities

Manage and nurture ongoing relationships with system partners, local GP Practices and Patient Participation Groups (PPG), this could involve attending meetings outside of core hours.

Be a proactive team player, manage their own time and workload and work under their own initiative.
