Job summary
Do you want to provide specialist leadership and support to our Community Service Teams who care for our patients, who are at the heart of everything we do at the Hospice?
If you are a Team Leader with a strong can do work ethic, registered with the NMC and have management experience/a management qualification and have worked previously within a Community care setting plus you have a full UK driving licence we are looking for you!
Do you have the ability to:
- Manage community based nurses and HCAs ensuring we provide a consistently high standard of palliative nursing care keeping up the reputation of the Hospice
- Work in collaboration with other community services, external organisations and local commissioners to promote and develop Dr Kershaws Community Services
- Work as part of a multi professional team, acting as an autonomous practionner, who continues to develop their knowledge base and clinical assessment skills, working within agreed practices and processes
- Use advanced communication skills to establish and maintain effective communication, with a wide range of individuals and groups about sensitive and complex matters
Main duties of the job
Manage all the Community Services on a daily basis having responsibility for the nursing and HCA team, providing specialist leadership and support for staff.
Working closely with the End of Life Educator to lead practice and educational development at departmental level.
Acting as an autonomous practitioner, continually developing a specialised knowledge base and clinical assessment skills, working within agreed processes and practices as part of a multi-professional team.
Support staff in the management of patients requiring palliative, end of life care.
Liaise with Community Palliative Care Nurses, GPs and Hospice medical colleagues (where appropriate) in order to assess, implement and facilitate appropriate treatment and care.
Work in collaboration with other community services, external organisations and local commissioners to promote Dr Kershaws Community Services. Maintain established working relationships and uphold the excellent reputation of the Hospice.
Support the Community Services staff to respond appropriately and support patient and family members in their own homes.
