CARE COORDINATOR - ALLOCATOR | Band 4 £26,530 - £29,114 per annum

Based at our Winsley office. You will manage safe and efficient scheduling of Patient visits independently and under direction of the Service Manager. You will also provide full administrative, secretarial and coordination support to Hospice at Home under the guidance of the Clinical Business Support Lead and Service Manager. Other duties will include:

  • Responsible for accessing and managing patient databases like Access and SystmOne, ensuring meticulous documentation and records upkeep to facilitate efficient operations and accurate patient information retrieval.
  • Act as the primary point of contact for receiving and documenting referrals to Hospice at Home, meticulously capturing all pertinent details and swiftly acting upon them to ensure seamless care coordination and timely response to patient needs.
  • Monitor NHS and departmental email accounts diligently, serving as a vital link between incoming inquiries and the relevant internal teams, ensuring effective communication channels and timely response to pending actions.
  • Provide regular reports on referral flow to the Service Manager and collaborate with leadership to assess capacity for new care packages, ensuring the Hospice at Home team remains responsive to community needs and achieves established performance goals.
  • Take a proactive role in planning and allocating assessors and care start dates, working closely with leadership to optimize resource allocation and ensure timely service delivery aligned with patient requirements.
  • Maintain vigilant oversight of visit plans and carer rosters, ensuring sufficient staffing levels to meet patient demand and making necessary adjustments to ensure seamless care provision.
  • Offer compassionate communication and support to patients and families, particularly during telephone interactions, demonstrating empathy, active listening, and providing appropriate guidance and referrals as needed.
  • Provide comprehensive support for financial processes, data collection, and reporting, contributing to ongoing service monitoring and evaluation, while upholding Dorothy House values and professional standards to foster a culture of excellence within the team.

  • Experience in working in an scheduling care visits within a community care service
  • Experience of working with patient databases. Able to record, analyse and interpret complex and sensitive information
  • Experience of working in a Community Social Care service
  • Excellent communication skills both verbal and written, with effective and sensitive telephone skills
  • Able to deal with and demonstrate sound judgment with sensitive, emotional and confidential situations.
  • Able to use initiative, prioritise and be proactive in all aspects of the role
  • Understands the importance of confidentiality and information governance
  • Able to demonstrate a flexible approach to hours and working patterns
  • Desirable requirements:
  • Experience producing reports and care invoices
