Job summary

The Summariser & Medical Coder will support on the development and delivery of a continual quality improvement plan, to improve the quality and safety for our population. This plan will be based on local intelligence and regional direction. To be responsible for the read coding of relevant information into patients electronic healthcare records on SystmOne clinical system, adhering to the organisation coding policy. In addition, the post holder will also support the multi-disciplinary team, providing administrative support as required, in line with the needs of the organisation.

The post holder will also work with the Nursing and Quality Team to deliver the quality assurance processes across all commissioned care, under the leadership the Quality Lead and Lead Primary Care Nurse.

The following are core responsibilities.There may be, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

a. Reviewing records and accurately producing a summary of the patients medical history

b. Accurately read coding data on SystmOne

c. Reviewing progress of summarising, ensuring practice targets are achieved

d. Review all clinical letters ensuring they are processed appropriately in line with extant protocols

e. File and store records as required

f. Photocopy documentation as required

g. Action all incoming correspondence, emails and mail in a timely manner

h. Review patient related documentation and attach scanned documents to patients healthcare records

i. Input data into patients healthcare records as necessary

j. Reviewing records and accurately producing a summary of the patients medical history

k. Reviewing records and accurately producing a summary of the patients medical history

l. Accurately read coding data on SystmOne

m. Reviewing progress of summarising, ensuring practice targets are achieved

n. Review all clinical letters ensuring they are processed appropriately in line with protocols

o. File and store records as required

q. Action all incoming correspondence, emails and mail in a timely manner

r. Input data into patients healthcare records as necessary

s. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately

The post-holder will implement and lead on a full range of promotion and management of their own and others health and safety and infection control as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include (but will not be limited to):

Using personal security systems within the workplace according to practice guidelines

Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines

Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangement

Making effective use of training to update knowledge and skills

Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of cleansing and decontamination equipment is sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate

Safe management of sharps procedures including training, use, storage and disposal

Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and safe way, free from hazards. Initiation of remedial or corrective action where needed or escalation to responsible management

Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised

Undertaking annual infection control training as per practice policy

Waste management including collection, handling, segregation, container management, ordering storage and collection
