Job summary
The homeless hospital discharge service provides enhanced inpatient care, and ensures safe, appropriate and sustainable hospital discharges that meet the needs of patients currently experiencing homelessness.
The service works with a variety of hospital, statutory and voluntary partners to achieve these objectives and aims to bring a wider influence to hospital, housing and social care system to proactively identify and better respond to the needs of currently homeless and otherwise disenfranchised people.
The overall aim of the Support Worker post is to provide a supporting role to work within the Bradford Homeless Discharge Team. The successful candidate will be responsible for the identification and assessment of homeless/unsuitably housed patients admitted to Bradford Teaching Hospitals alongside other team members. This will include attendance at multi-disciplinary meetings and the need to provide community support to patients discharged from hospital.
They will be expected to work closely with the Lead Nurse and Homeless Discharge Team. The team works closely with staff across all disciplines between hospital and community to ensure a safe, timely and effective transfer of patient care on discharge.
Communication and team working is an essential aspect of this role. The successful candidate must be able to work both autonomously and well within a small team.
Main duties of the job
Be responsible for the identification of homeless/vulnerable patients who attend A&E and are admitted to wards alongside the other team members.
Carry out a holistic assessment in order to address both physical and social needs, subsequently assisting in the setting up of support packages based on their assessment. This will include support with the patients housing, physical health needs, alcohol and drug dependencies, mental health issues and any other social care needs that the patient would benefit from been supported with.
