Job responsibilities

JOB PURPOSE: To provide primary medical care for homeless people, people in insecure accommodation, refugees, asylum seekers and all other patients registered at Bevan House Primary Care Centre (the Practice), at the practice, and to contribute to the development of health and other services for these groups

LOCATION and hours of work:

Clinical work will take place at the practice. Hours may be flexible according to service need.

Roles and Responsibilities

Clinical

Deal promptly with patients problems, providing usual primary care based treatment and referring as appropriate

Offer full preventive health care services

Work closely and co-operatively with other members of the practice team and partner organisations, providing advice and support when required, particularly in the area of substance misuse.

Work to ensure that the practice achieves its quality outcomes and contractual targets

Keep careful medical records, and collect data required for health needs assessment, and for monitoring practice performance

To act as an advocate for patients when appropriate.

To ensure data protection guidelines are followed, confidentiality is respected and patient access to services is promoted

To promote patient self-care through education and support a preventative approach

Service Development

Be involved in audit, service review and if appropriate research.

Contribute to the development of services in Bradford for people who are homeless or in insecure accommodation, and for refugees and asylum seekers.

Provide leadership and clinical governance input for specified clinical areas.

Promote evidenced based practice and the dissemination of good practice amongst colleagues and peers

Participate in a culture that prioritises quality and safety

Help fulfil the practices aims and objectives, as expressed in the Bevan Healthcare vision and mission statement

Educational

Maintain a Personal and Professional Development plan, and keep well informed about developments in care for the particular client group of the Practice

Contribute, as appropriate, to the delivery of inclusion health training provided by Bevan Healthcare.

Participate in the Practices in house programme of education, training and development

Participate in the Bevan Healthcare in house appraisal programme

To contribute to the development and dissemination of best practice

Administrative

Write referral letters and reports as needed, and perform other appropriate general and personal medical services administrative work

Contribute to the development of practice communication and administrative systems, and to the development of the practices Information Management and Technology systems

Comply with Bevan Healthcare CIC Clinical Governance Standards and Policies and Procedures.

Be responsible for record management and security in accordance with information governance guidance

Person Specification

Experience

Essential

• Excellent verbal and written communication skills
• Good team working skills, IT and audit skills
• Ability to work independently & under pressure
• Understanding of the health problems facing homeless people and other vulnerable groups
• Confidence in the management of physical and mental health problems
• Evidence of continuing professional development, personal development & service development

Desirable

• Minimum 2 years experience of general practice post completion of vocational training
• Experience of inner city general practice within a multi cultural context
• Experience of work with asylum seekers/refugees
• Experience of service evaluation and/or research
• Foreign language skills
• Experience of work with homeless people
• A minimum of 2 years experience in the provision of medical assessment and treatment of substance misuse problems
• Teaching skills
• Interest in or experience of adult safeguarding
• Experience of undertaking clinical governance

Personal qualities

Essential

• Enthusiasm to be involved with inner city general practice
• Able to handle conflict and stress
• Flexible approach

Desirable

• Possession of a full driving licence

Qualifications

Essential

• Primary medical degree
• Full registration with the General Medical Council
• Completion of vocational training in General Practice, with certificate of prescribed or equivalent experience from the JCPTGP or written evidence of exemption from the need for such experience

Desirable

• Membership of or aspiration to membership of the Royal College of General Practitioners
• RCGP certificate parts 1 and 2 in substance misuse or equivalent
