Job summary

Bevan Healthcare CIC is a social enterprise committed to improving the health of the most vulnerable groups through their practices in Bradford and Leeds. We are widely recognised as pioneers in our field and have received an Outstanding CQC Rating. We are currently seeking to recruit a GP to our Bradford and Leeds Practice.

Main duties of the job

The post offers:

Delivery of primary care within a health and social model

Fascinating work with a diverse patient group, with a list size of approximately 6500 over both sites

A supportive, experienced team, highly committed to inclusion health

Additional street medicine option the opportunity to provide healthcare to the homeless and other vulnerable patients at outreach locations

Regular, paid education sessions, focused on key, relevant topics, plus 1 week per annum study leave (pro rata)

15 minute appointments

Job description

Job responsibilities

JOB PURPOSE: To provide primary medical care for homeless people, people in insecure accommodation, refugees, asylum seekers and all other patients registered at Bevan House Primary Care Centre (the Practice), at the practice, and to contribute to the development of health and other services for these groups

LOCATION and hours of work:

Clinical work will take place at the practice. Hours may be flexible according to service need.

Roles and Responsibilities

Clinical

Deal promptly with patients problems, providing usual primary care based treatment and referring as appropriate

Offer full preventive health care services

Work closely and co-operatively with other members of the practice team and partner organisations, providing advice and support when required, particularly in the area of substance misuse.

Work to ensure that the practice achieves its quality outcomes and contractual targets

Keep careful medical records, and collect data required for health needs assessment, and for monitoring practice performance

To act as an advocate for patients when appropriate.

To ensure data protection guidelines are followed, confidentiality is respected and patient access to services is promoted

To promote patient self-care through education and support a preventative approach

Service Development

Be involved in audit, service review and if appropriate research.

Contribute to the development of services in Bradford for people who are homeless or in insecure accommodation, and for refugees and asylum seekers.

Provide leadership and clinical governance input for specified clinical areas.

Promote evidenced based practice and the dissemination of good practice amongst colleagues and peers

Participate in a culture that prioritises quality and safety

Help fulfil the practices aims and objectives, as expressed in the Bevan Healthcare vision and mission statement
