Job summary

An exciting opportunity has arisen to join the TRANSFORM Team to make a difference to the care of those in the last months of life in hospital, care homes and at home by educating, supporting and empowering clinical staff, families and patients.

Main duties of the job

The successful candidate will be a registered nurse with previous senior level experience of end of life care. The post-holder will be expected to work their hours over seven days a week in all care settings.

  • - Attendance at 6 day Principles and Practice of Palliative Care Course (Terence Burgess Education Centre at Queenscourt) or equivalent

Desirable

  • - Palliative Care Qualification
  • - Advanced Communication Skills Training
  • - Educational qualification

Experience

Essential

  • - Senior level experience of the end of life care of patients and families
  • - Management experience
  • - Education experience
  • - Proven experience of use of the key enablers of end of life care
  • - Audit and report writing experience

Skills & Knowledge

  • - Ability to manage own time
  • - Accurate in literacy and numeracy

  • Special interest in working in palliative care. Highly motivated, can demonstrate initiative and an intuitive approach to dealing with sensitive situations. Willingness to be hands on and ability to mix with all sections of the community.
