Job summary

Are you an Occupational Therapist or Physiotherapist who is looking to develop your skills in Palliative Rehabilitation and specialising in a role within St Barnabas Hospice?  We have two positions as follows:

Our new Wellbeing Hub will be opening later this year and the successful candidates will have the opportunity to develop the service for the people of Boston which will be the focus for one post, with the other focusing on providing care in North Lincoln/Lincolnshire.

This is an exciting opportunity for autonomous, highly motivated and dynamic Physiotherapist to work within St Barnabas Community Services. You will work within a supportive Multi-Disciplinary Team - delivering Palliative Rehabilitation to those patients with a range of Complex Symptom Management needs

Qualifications and Experience:

Applications are welcome from candidates with:

  • Degree (BSc/MSc) in Occupational Therapy or Physiotherapy, with current registration with the HCPC
  • Experience with management of Respiratory, Oncological, Frailty, Neurological Conditions
  • Recent post-registration experience in a relevant area; i.e. Acute Medicine, Neurology, Oncology, Palliative Care or Community Therapies

Main duties of the job

You will be able to:

  • Develop Palliative Rehabilitation and Occupational Therapy/ Physiotherapy Practice in line with current and appropriate Evidence Base
  • Perform the role with empathy and consideration for the needs of a range of patients and their families
  • Adapt effectively to varying levels of demand and to support less experienced staff to maintain high levels of care

Job Purpose

To complete comprehensive assessments of patients with complex Palliative and Life Limiting Conditions and deliver a high standard of specialist Palliative Rehabilitation to patients across Lincolnshire.

To provide as a member of a multidisciplinary / multi-agency team and acting as an autonomous practitioner, effective, evidence based clinical assessment to patients referred to St Barnabas Community Services. This will include assessments, diagnosis, planning, delivery and evaluation of treatment, regularly reviewing and updating of care plans and documentation as determined by patient need / care pathway.

To plan, co-ordinate, deliver and evaluate the Physiotherapy treatment programme for patients with palliative care needs. The care provided will be appropriate to the patient’s needs and stage of illness and use a rehabilitation model to promote quality of life.

The post holder will be able to respond to situations for patients known to the service to prevent admission to hospital and work effectively under challenging physical, social and psychological environments where patients/relatives/carers may exhibit high levels of distressed behaviours.

To contribute to Specialist Palliative Care and Palliative Rehabilitation education within the organisation and externally.

To direct interventions delivered by support staff and volunteers - both individual and group work, with patients through documented plans of care, training and supervision.

To maintain records regarding assessment and treatment of patients as indicated by the relevant Trust policy.

All aspects of practice must be in line with HCPC and Chartered Society for Physiotherapy (CSP) guidance.

There may be a requirement to assist in the delivery of services at other bases across the county and in other care settings – such as patients own homes, care homes and the wider community.
