Job summary
We
have a vacancy for a Health Rehabilitation Support Worker to work as part of
the South East Community Service Team. You will be expected to work alongside
other healthcare professionals and volunteers to provide high standards of
specialist palliative care and psychological support for patients, families and
carers within the day therapy and hospice at home setting.
Main duties of the job
Qualifications and Experience:
- Applications are welcome from candidates who understand the core values of care and compassion in daily practice and can demonstrate excellent communication skills.
- Basic computer skills and understanding of the principles of confidentiality are essential.
- NVQ level 2 in Health and Social Care or equivalent and skills in palliative rehabilitation are desirable.
- This role requires a DBS check.
Contract: Hours negotiable, up to 22.5 per week
Salary: £18,925 per annum pro rata, (plus weekend unsocial hours enhancement of 30% and contributory pension scheme or continuation of NHS pension)
Buying and selling of annual leave and lease car schemes are options available to qualifying new employees.

- Demonstrate basic IT skills e.g. able to use a computer and applications (MS Outlook etc.)
- Ability to observe and effectively report changes in patient condition
- Experience of delivering rehabilitation programmes under supervision of Occupational Therapists or Physiotherapists

- Sufficient ability in English to accurately provide and receive information verbally and written
- Ability to perform the role with empathy and consideration for the needs of patients and their families.
