Job summary

Working as part of the governance and education team, the role of the ECHO project Manager is to support the delivery and roll out a commissioned contract for project ECHO across the County of Lincolnshire. The scope of the project will support improving end of life care outcomes by working with colleagues across the health and social care system in Lincolnshire.

Project ECHO is the extension of Community Healthcare Outcomes, an established not-for profit tele mentoring project which was developed in the USA to improve care of patients. It involves the use of cloud-based networking to connect specialist HUBS and SPOKES to create a community of practice, sharing knowledge and which supports service delivery.

Main duties of the job

The emphasis will be on enhancing the delivery of services that are clinical through the use of ECHO by extending the reach of Hospice Care, so Hospice care and principles can be delivered in any setting. The post holder will therefore require a registered clinical qualification to provide expertise, support and influence to deliver the project. There will be a requirement to negotiate and work alongside key suppliers/stakeholders and internal staff, for example the Hospice IM&T, and Marketing team.

Qualifications and Experience:The post holder will require a registered professional qualification and have excellent organisational and communication skills with the knowledge of delivering clinical projects across different settings and external providers across the County.

About us

The post holder will be responsible for the day to day management of the project this includes , managing the two dedicated administrators, in addition the role will support other works teams associated with the project to ensure that the project is delivered to agreed expectations, on time and within budget. The role requires excellent IT and project management skills including Microsoft Excel, Office, PowerPoint, intranet and internet and databases to extract and model and manipulate data to produce complex written, verbal, tabular and graphical information.Base: Countywide, base to be confirmed

Job Summary

To lead on and manage Project ECHO and other workstreams related to the project, including working with specialist physicians to support and develop the end of Life Care Website for Lincolnshire, and countywide embedding of EPaCCS.

Ensure that project aligns to the organisations financial envelope for delivery of the project and that any associated risks to the delivery of the project are identified and reported appropriately.

Build and strengthen relationships with key stakeholders both internally and externally to ensure a proactive approach is adopted to managing the project.

Develop robust communication mechanism with key stakeholders, both internally and externally to ensure that effective communication processes are embedded.

To participate in Project ECHO sessions and Team meetings.

Key Working Relationships

The post holder will be required to maintain constructive relationships with a broad range of internal and external stakeholders such as member GP practices, Lincolnshire Clinical Commissioning Group, Neighbourhood Teams, Commissioning Support Unit and Providers external to the County.

Provide coordination of, and participate in, relevant internal and external working groups and provide project advice, expertise and support where requested. Where relevant, ensure that the views of patients or other service users are taken into consideration, including representation if required.

Present information and issues, explaining highly complex issues, to a wide range of internal and external stakeholders.

To liaise with other Managers across Lincolnshire Clinical Commissioning Group to share best practice.

To negotiate with, liaise with, co-ordinate and monitor the activities of suppliers/stakeholders and internal staff, for example IM and T , marketing , as appropriate to ensure all projects are implemented in a timely manner, to a high quality and to agreed standards.

Develop and maintain links with Hospice UK project ECHO Programme Leads.

Project Management

Provide relevant and timely updates advice and guidance on the project delivery milestones and the projects function, both internally and externally.

Work with key delegates internally and externally to investigate the root causes of any variance from target/plan and proactively contribute to the implementation of solutions.

Management of project-associated risks and issues including tracking mechanism and its proactive resolution and escalation of processes.

Produce project progress reports and identify key project information to support the maintenance of the project, including Dashboard and RAG report.

To communicate, publicise and promote project activities and progress to appropriate staff internally and externally.

Leadership and Management

Responsible for the overall management of the Project ECHO Coordinators, ensuring delivery of the Project ECHO educational sessions.

Lead and line manage the Project ECHO Coordinators to deliver the Project ECHO sessions, to engage and support all participates with the agreed learning outcomes.

Ensure the Project ECHO Coordinators meet the Trusts relevant Performance Indicators, monitoring effectiveness through the team.

Ensure the Project ECHO Coordinators adhere to the Trusts rules and regulations for Health and Safety. Actively influence staff engagement within the team to ensure it is maintained. This includes:

Appraisals being completed by due date, and personal development plans put in place as appropriate.

Sickness absence being managed in line with the Trusts absence policy.

Probationary process being managed in line with the Trusts policy.

Ensuring staff are regular updated through team meetings or other channels and there are forums for two-way communication.

Lead by example and ensure that behaviour is in line with the Core Values of the Trust.

In conjunction with the Governance and Educational Lead, develop and work towards an integrated patient and family support and well services strategy which will support the Trusts strategic objectives.

Financial/Physical Resources

Oversee and monitor all aspects of the budget associated with project. regular providing updates on spend and forecast escalate any financial risks.

Information Management

Responsible for the management of information in accordance with the General Data Protection Act 2016, ensuring procedures are developed for the lifecycle of the electronic information utilised within the project.

To manage the data flow of information and use this to analyse and report to enhance decision making processes.

Audit ,Research and Development

Contribute to the development of key performance indicators for the successful delivery of the project.

Undertake research and development activities related to the project, collating information, analysing and reporting findings.

Support Identification of key priorities pertaining to the project , including audit across the health community using both national and local intelligence.

Planning and Organisation

Contribute to the strategic planning of projects, identifying interdependencies across projects/functions, potential impacts on wider organisation, resource requirements and building in contingency and adjustments as necessary.
