As a Care Coordinator, youll play a vital part in improving patient care by supporting people especially the elderly and those with long-term conditions to navigate the health and care system with confidence.
Main duties of the job
Your role: Work closely with GPs, practice staff and wider PCN teams Act as the main point of contact for patients and carers Help develop and coordinate personalised care plans Refer to services like social prescribing and wellbeing coaching Support patients before and after clinical appointments Empower individuals to understand and manage their health This is not a clinical role, but its central to delivering high-quality, joined-up care that puts people first.
