Join us at the start of an exciting, new, and challenging healthcare initiative working with South Hardwick PCN Acute Home Visiting Service (Team Up).

South Hardwick PCN are looking for a Nursing Associate to work alongside Community GPs, Advanced Clinical Practitioners, and other members of a multi-disciplinary team to provide healthcare to our housebound and care home patients across the PCN footprint.

This is an evolving service, so you will have the opportunity to contribute to the development of Team Up! and the Home Visiting Service.

The aim for the service is to ultimately provide a holistic approach to acute on day/rapid response services, enhanced health in care homes and enhanced proactive care for older people with frailty and patients with multi-faceted health problems.

You will be part of a team that leads on medical models of care across primary and community-based settings, in collaboration with multi-professional and multi-organisational colleagues. You will also play a crucial role in step up, step down and anticipatory care through timely assessment, referral, responsive intervention, and care planning.

The post holder will be required to travel across the South Hardwick PCN area, and will need a valid UK driving licence and own transport.

Main duties of the job

  • Work as part of Team Up's MDT to provide and monitor care, under direct or indirect supervision
  • Support registered nurses to enable them to be able to focus on the more complex clinical care
  • Develop relationships across the PCN to support integration of the role across health and social care including primary care, secondary care, and mental health
  • Perform and record clinical observations such as blood pressure, temperature, respirations, oxygen saturations and pulse
  • Promote health and well-being to all patients, for example undertaking the NHS health checks
  • Support the delivery of services to enhance care to care homes, the housebound and frailty population through virtual, telephone, face to face and home visit reviews
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services
  • Care for individuals with dementia, mental health conditions and learning disabilities
  • Chaperone patients when required
  • Maintain accurate and contemporaneous patient health records
