It will involve true MDT working, building relationships with GP practices, Community Rapid Response services and Falls services, to help clinically deliver the service on a day-to-day basis.

This is an evolving service, so you will have the opportunity to contribute to the development of Team Up! and the Home Visiting Service.

Main duties of the job

South Hardwick PCN are looking for a Lead Advanced Clinical Practitioner to work alongside Community GPs and other members of a multi-disciplinary team to provide healthcare to our housebound and care home patients across the PCN footprint, and to provide clinical leadership and support for the team of ACPs delivering the AHVS.

You will be part of a team that leads on medical models of care across primary and community-based settings, in collaboration with multi-professional and multi-organisational colleagues. You will also play a crucial role in step up, step down and anticipatory care through timely assessment, referral, responsive intervention, and care planning.

You will also contribute to the development of Team Up & the Home Visiting service working with other clinicians.

About us
