Job summary

Join us at the start of an exciting, new, and challenging healthcare initiative working with South Hardwick PCN Acute Home Visiting Service (AHVS), also known as Team Up.

South Hardwick PCN is seeking Administrators to work alongside and support a multi-disciplinary team, to provide care to our housebound and patients residing in care or residential homes.

We are looking for experienced, enthusiastic, and motivated people who are flexible and adaptable to work as Acute Home Visiting Service Administrators.

This is an exciting opportunity to be at the forefront of the delivery of our new Team Up/Ageing Well model of care. The role will operate across the traditional health and social care organisational boundaries, including with our GP practice partners, Community Rapid Response services and Falls services, ambulance and out of hours services to help clinically deliver the service on a day-to-day basis.

The aim for the service is to ultimately provide a holistic approach to acute on day/rapid response services, enhanced health in care homes and enhanced proactive care for older people with frailty and patients with multi-faceted health problems.

Main duties of the job

You will:

Have excellent administration, secretarial and communication skills to maintain high standards of work. You will be confident in working under pressure and be able to work on your own initiative.

Be the first point of contact for the team, taking telephone calls from practices/accepting referrals for home visits, and ensuring these are logged on the system and directed to a Clinician for triage.

Provide a full and comprehensive administrative service to support the team, and project a positive, friendly and empathetic image to patients and other visitors.

Establish administrative systems and processes to maintain effective working within the team.

Support the team with data collection and analysis to provide evidence for monitoring and evaluation purposes.

Develop and maintain constructive relationships with internal and external stakeholders.

Facilitate effective communication between patients, the AHVS team and other health and social care organisations.
