Hope Citadel Healthcare are a not-for-profit social enterprise that believes General Practice can be at the heart of transforming individuals, families and communities. We hold the contracts for 9 Greater Manchester practices (with 3 Outstanding ratings), and are looking to expand our team of committed clinicians.
This is a new Remote GP role we are introducing to adapt to the changing landscape of Primary Care since the pandemic. This means we can consider a broader range of candidates some of which may not be within a commutable distance from our practices. We will also consider candidates who may wish to work on site on days that work for them or to attend clinical meetings etc.
We support doctors with 13 minute appointments as standard. We have specialist deprivation workers working alongside practice nurses, in house counsellors and admin team to unpick the complexity of clinical, social and psychological problems our patients face. Main duties of the job
The applicant will undertake all the normal duties and responsibilities associated with a GP working within primary care although different from a regular on site GP with the weighting of Admin/Patient contact being around 70/30.
Including managing a caseload and dealing with a wide range of health needs, ensuring the highest standards of care for all registered and temporary patients.
Undertake a variety of duties including, telephone consultations and queries, triage calls, checking and signing of repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
Develop patient care plans with patients in line with current Practice disease management protocols
To assist in the establishment of appropriate systems to manage common chronic medical conditions.
Completion of clinically related administrative and non clinical duties needed for the delivery of the service.
Refer patients for further opinions and interventions according to Practice protocols
Recording clear and contemporaneous consultation notes to agreed standards.

Your role will involve working across our 11 Practices, the practices cover Heywood, Middleton, Rochdale, Oldham, Shaw and Levenshulme with an average of 4600 patients per practice.

A remote clinical shift will constitute of 3 hours of Admin and
1.25 hours of virtual/telephone appointments. These will be 13 minutes in
length.
As this is a newly created post the role may evolve to fit the service needs in collaboration with the successful candidate. You will be given access to the clinical systems at our 11 sites and depending on where the need is most you may be asked to focus your efforts on those specific practices.
In general the post-holder will be expected to undertake all the
normal duties and responsibilities associated with a GP working within primary
care. Barring those that require you to be on site to complete.

recording requirements under QOF and Enhanced Services.

in the collection and collation of statistics towards the preparation of
practice reports, performance monitoring and audits
Participating
in the auditing of practice activity
Process and
action incoming patient and hospital correspondence
Attendance at
in-house meetings, significant event audit meetings and others as deemed
necessary
Ensure that the
practice complaints system is adhered to at all times
Awareness and
compliance with all relevant guidelines e.g. data protection, confidentiality
and health and safety
Maintain
current CPR and anaphylaxis certification
Management of Risk
Manage and
assess risk within the areas of responsibility, ensuring adequate measures are
in place to protect staff and patients

to ensure they are safe and free from hazards and conform to health, safety and
security legislation, policies, procedures and guidelines

mandatory and statutory training

qualified. In this instance a set number of patient appointments will be
amended to education/supervision slots
