Glodwick Medical Practice in Oldham is looking for a Practice Nurse. This is a growing practice with a large and diverse population, rated Good with an Outstanding by CQC.
We are looking for an experienced Practice Nurse to join our friendly team as we look to better serve the needs of our community. You will be supporting patients to be healthy, by monitoring long term conditions, and by focusing on health prevention and screening. You will be working alongside a team of GPs, a Focused Care worker, HCA, Specialist Nurse and a brilliant admin/reception team.
Main duties of the job
- Assess, plan, develop, implement and evaluate programmes to promote health and well-being and prevent effects on health and wellbeing.
- Implement and evaluate individual treatment plans for patients with known long-term condition or at risk of a long term condition.
- Support and manage patients requesting information relating to family planning needs
- Support and manage patients presenting for cervical cytology consultations
- Implement and participate in vaccination and immunisation programmes for both adults and children
- Advise, support and, where appropriate, administer vaccinations for patients travelling abroad
- Assist senior practitioners in providing minor surgery sessions

Job Summary
The post holder is
responsible for the delivery of basic practice nursing services to the practice
population. Supported by the practice team, they will deliver care and focus on
supporting patients to be healthy, monitor long-term conditions, health prevention
and screening activities. They will work collaboratively with the general
practice team to meet the needs of the patients, supporting the delivery of
policy and procedures.
