Advanced Nurse Practitioner for Care Homes and Frailty

Job summary

The post holder is an Advanced Nurse Practitioner (ANP), caring for people who are Frail and/or are housebound or live in a care home. They will work for Canalside PCN, made up of 6 GP practices in Heywood Middleton and Rochdale. They will work across sites with a focus to work as part of a multidisciplinary team caring for these patients.

The ANP will be responsible for proactive, anticipatory care and case finding as well as managing and coordinating palliative and frailty care, mainly within the Care homes within Canalside PCN

Acting within their professional boundaries, the post-holder will provide care for the patient following treatment plans and evaluations of their care. They will demonstrate safe clinical decision-making and provide expert care for patients.

They will work collaboratively with the multi-disciplinary General Practice team and the PCN team including Advanced Paramedic, Nurse Associate and Clinical Pharmacist to meet the needs of patients, supporting the delivery of policy and procedures. In order to work at this level, NMC requirements for Advanced Practice must be met.

Main duties of the job

Deliver a high standard of patient care as in Nursing, and Residential Care Homes and for patients of the Practice and PCN with Frailty and who are housebound with a focus on their palliative needs.

Manage a clinical caseload, dealing with patients needs to include frail, palliative and vulnerable patients registered with the PCN.

Instigate and be involved in pro-active, case finding and anticipatory care and appropriate advanced care planning discussions with all patients and their families and develop personalised care plans.

To be part of an MDT responding to correspondence and clinical administration pertaining to patients within the role-holders scope of work.

Build relationships between all agencies, working collaboratively coordinating and/or advocating for patients' care.

Mentor, support other Health Care Professionals in developing and maintaining clinical skills and knowledge.

Canalside PCN is a high acheiving group of 6 GP practices who, work together to deliver good patient care. We work collaboratively together and with the local ICB in HMR. We are expanding our Multi-disciplinary team of PCN roles to meet the needs of our patients and care homes.

Professional Role To promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research. To maintain their professional registration and keep up to date with mandatory training. To record accurate consultation data in patients records in accordance with the latest regulatory guidance and other pertinent standards. Pro-actively promote the role of the ANP. Clinical Role To assess patients within their roles and make professionally autonomous decisions. Provide a first point of contact alongside the Frailty and Care homes Care Coordinators for Nursing and Residential Care Homes. Provide regular ward rounds and telephone calls / visits to Care Homes and anticipatory care visits as per the post-holders case load. Instigate necessary diagnostic tests or investigations where appropriate and interpret findings/reports and act upon them appropriately. Reviewing patients where needed and making long term care plans. As an independent prescriber: to prescribe safe, effective and appropriate medication as defined by current legislative framework. To work with the medicines management team to provide and/or ensure polypharmacy medication reviews and de-prescribe where appropriate. To provide safe, evidence-based, cost-effective, individualised patient care. To provide advanced care and respect planning with a focus on Palliative and End of life care where appropriate. To refer patients directly to other services/agencies as appropriate. To work with the MDT to ensure practice and PCN contractual requirements are delivered and met for patients within caseload and in support of our wider population. To work with the MDT to ensure our care is compliant, safe, of high quality and follows regulatory guidelines at practice and PCN level.
