Job summary

This is a great opportunity for a Registered Physiotherapist to join the Supportive Care team at Hospiscare, where they will work alongside other medical professionals as part of the multi-disciplinary team delivering excellent evidence based palliative care. Applicants must have substantial experience of working with patients who have life-limiting conditions, and significant experience of planning, assessing and evaluating treatment and care plans.

Main duties of the job

To be considered, you will need to demonstrate detailed understanding of the complexities of working with individuals at times of distress and conflict and of theoretical nursing models and their impact on delivery of care, while an understanding of key issues relating to End of Life care would be beneficial. Highly developed communication and interpersonal skills, and developed IT skills, are also essential to be successful in the role, as is the ability to lead and work as part of the team, prioritising workload and managing time effectively.

As you would also assist the Supportive Care Team Leader with clinical leadership and supervision of junior colleagues, experience of coaching/mentoring and of facilitating clinical supervision is desirable.

2. JOB PURPOSE

To work as part of the multidisciplinary specialist team delivering an excellent standard of evidenced based palliative care.

To promote dignity, encompassing a person centred approach, to positively enhance the lives of patients with complex life limiting illnesses.

3. KEY RESULT AREAS/PRINCIPAL DUTIES AND RESPONSIBILITIES

3.1 Main Responsibilities

Provide advanced specialist palliative care, using experienced clinical knowledge and judgement in complex palliative and end of life care, to promote best practice for adults with life limiting illnesses.

Assess and review patients, prioritising needs and working as a member of the multi-disciplinary team, to ensure effective communication is in place to support patient care.

Undertake specialist physiotherapy assessment and treatment skills in all aspects of community and hospice based palliative rehabilitation, working as an autonomous practitioner in order to support the work of the multi-disciplinary team.

Assist with the development of a holistic needs assessment clinic, symptom management groups, exercise sessions and other initiatives including virtual clinics/groups in order to provide early access to therapy assessment in the day centre setting.

Demonstrate advanced communication skills and professionalism in all interactions with patients and those close to them, carers and other professionals in order to promote and maintain Hospiscare values.

Attend and contribute to multi-disciplinary team meetings to promote best practice, representing the Supportive Care service and/or individual patients to ensure the delivery of a coordinated multi-professional service and fully integrated therapy treatment plans.

Demonstrate knowledge and skills in contributing to the delivery of education and learning for all those involved in palliative/end of life care in order to act as a resource to wider professional and care teams both internally and externally.

Contribute to Hospiscares clinical governance and audit process in order to support continuing quality improvement and learning.

Supervise junior colleagues, contributing to daily clinical leadership and taking responsibility for the team in the absence of the SCTL, to ensure team effectiveness, cover and cohesion, continuity of service and the welfare of patients.

Assist the multi-disciplinary team in planning discharges to help patients achieve their preferred place of care.
