Job summary

To provide the highest possible quality of specialist medical palliative care and advice within the resources available in conjunction with the medical and multidisciplinary teams.

Main duties of the job

We are currently offering a fixed contract until August 2024. This is a great opportunity to gain palliative care experience within a dynamic well established hospice team. It would be great for professional development either for individuals with general practice experience or individuals looking to develop a career in palliative care.

About us

St Michaels is a newly refurbished consultant led hospice. Alongside the inpatient unit it has day services, outpatients and a Hospice at Home service all built on strong MDT and partnership working with our NHS and local authority colleagues.

St Michaels works closely with hospital and community generalist and specialist services to deliver its innovative and ambitious care strategy.

Continuous Professional Development (including ST3 GPVTS and FY2 training) is the corner stone of this experienced medical team.

Responsible for: Supervision of doctors in training, Foundation Year 2 (FY2) and GPVTS doctors.

Job Purpose:

To provide the highest possible quality of specialist medical palliative care and advice within the resources available in conjunction with the medical and multidisciplinary teams.

Responsibility Areas:

Direct provision of high-quality specialist palliative care

Assessment, diagnosis and management of patients under the care of the Hospice. Majority of work is based in the in-patient unit and/or Day Hospice, but if required as outpatients or domiciliary visits. Work includes:

Admission assessment and forming a management plan, providing explanation/ information according to individual patient preference, communication with patients/ family and other professional colleagues, arranging and reviewing appropriate investigations, safe and effective prescribing in accordance with prescribing standards, supporting advance care planning, managing challenging symptoms and with others providing leadership in challenging situations.

Providing effective documentation and handover.

Managing expectations and with others working to provide holistic high quality responsive general medical and specialist palliative medical care in accordance with current evidence, accepted practice and guidance.

Use of a variety of clinical support systems to aid clinical decision making.

Working within the medical team to maintain continuity of care.

Working with the nursing and multidisciplinary teams to provide effective joint care.

With the nurse in charge and ANP each day identifying patients for medical/nurse review or shared care.

Following the shared care procedures.

Assessment of appropriateness of discharge, contributing to discharge and aftercare planning.

Liaising promptly with professional colleagues involved in patient and family care during and after admission.

Contributing to admission prioritisation.

Using effective joint working and prioritisation to support responsive admissions within available capacity each day.

Liaison and provision of specialist palliative care advice

With professionals, patients and carers during working hours and on call. This includes decision-making over the appropriateness of admission or discharge to/from the Hospice.

Dictation of medical correspondence and use of NHS mail to liaise with professional colleagues.

Multi-disciplinary team working

Working with the multidisciplinary team to provide holistic specialist palliative care to patients/carers and families.

Ensuring appropriate involvement of relevant professionals, interfacing and planning most effective joint working.

To work alongside the wider multidisciplinary team in promoting the Rehabilitative Palliative Care approach enabling people to live fully until they die.

Share in the training and supervision of the FY2 and other trainees on rotation at the Hospice and providing support to others on educational attachment on the IPU.

Contribute to clinical governance and the Hospice Care strategy implementation.

Involvement in critical incident reporting, quality improvement activity or clinical guideline review is expected e.g. contribute to the medical team quality improvement schedule.

Contribute to the IPU working group and other work to develop hospice care services.

Follow professional code of practice and Hospice professional expectations, policies and procedures. Maintain compliance with mandatory training and mandatory reads.

Continuing professional development

Disseminate knowledge and skills at every opportunity throughout the Hospice multidisciplinary team and where appropriate external health care professionals.

Responsible for maintaining own professional development using reflective practice, allocated time for CPD and study leave effectively.

Compliance with GMC requirements to maintain revalidation and engagement with effective annual professional appraisal.

Dimensions and limits of authority/influence

Need to make safe diagnostic assessment and clinical management decisions including prescribing, appropriate investigation, interventions, communication and provision of specialist palliative care advice.

Assessment and decisions are made within personal competency with access to a senior medical officer or consultant for advice, assessment and supervision with twice weekly consultant ward round.

When on call a consultant is available for telephone advice. The consequence of an error is potentially very serious including death of a patient, serious harm, medico-legal claim against the Hospice, complaint and damage to Hospice reputation/income.

Clinical decisions are made on a constant basis with a need to prioritise competing demands.
