Main duties of the job

Youll lead a team of Occupational therapists, physiotherapists, wellbeing support workers and potentially other allied healthcare professionals.

At Community Hospice, were proud to be a trusted local charity. Our multidisciplinary teams work across community in patients homes, on our inpatient unit, through outpatient clinics and at our local hospital. Wherever people are, were there to help them live well for as long as possible.

Job responsibilities

You'll lead the Rehabilitation and Wellbeing Team in delivering high quality palliative rehabilitation and wellbeing support to patients and carers. This isn't just about care its about helping people live fully, with dignity, for as long as they can. You'll bring a holistic approach to support people to live as well as they can for as long as they can.

You'll work alongside colleagues from a range of disciplines within the hospice on the inpatient unit, outpatient and community services and advise the hospices hospital-based team. Sharing your expertise to help us all deliver the best care we can.

You'll build strong, honest relationships with partners across the health and care system because great end-of-life care doesn't happen in isolation.

Key Responsibilities:

Clinical Practice & Leadership

You'll work with the team to deliver care that is as individual as the people we support, always holistic and person centred. As the Lead for the Rehabilitation and Wellbeing Service, you will be a key member of the team, providing evidenced based clinical expertise and leadership and will:

Lead and champion the service making sure it is seen, heard and valued

You'll build strong, honest partnerships with colleagues working in other services because great care doesn't stop at the hospice

You'll lead your team with clarity and care- supporting their growth, guiding their work and making sure they feel confident and capable every step of the way

You'll help people hold on to what matters mobility, independence, dignity, social networks by creating care plans that are practical, empowering and personalised

You'll lead by example showing what compassionate, skilled care looks like and lifting others along the way

You'll work closely with senior colleagues to shape and grow the service, bringing your experience and ideas to the table

  • In depth experience of legislation and policy surrounding mental capacity and additional specialist provision
