Job summary

Weymouth and Portland PCN are offering an exciting opportunity to develop and optimise the management of cardiovascular disease and to reduce health inequalities in this pilot project.

This is an initial 18 month project looking at the early identification of hypertension and atrial fibrillation, improving patient education and optimizing management particularly looking at reducing health inequalities in access to care. It is anticipated that the positive outcomes that we expect to see from this project may allow this to be continued at the end of the project but this will be contingent on proven outcomes.

Main duties of the job

We are looking for a Nurse with chronic disease management skills in cardiovascular disease. As part of this role will be delivering education sessions you will also need to be confident in presenting to groups of patients.

You will be responsible for helping to design the service in partnership with the PCN leads in cardiovascular disease & inequalities and the PCN Clinical director. Administrative and digital support will be provided.

Hours are negotiable up to full time but there will be occasional evening working and potential occasional Saturday clinics in order to reach the target populations.
