Job summary

We are looking for a part time/sessional Physiotherapist (2 half days or 1 whole day) to join The Weymouth & Portland Frailty Service. This is a well established multi-professional community team working to improve care for our frailer patients. All the practices in Weymouth & Portland are working together to provide this service. The Physiotherapist will join a team consisting of 2 GPs and 6 Frailty Practitioners and an occupational therapist, with support from 2 Frailty Assistants (HCAs) to proactively manage frail patients. Admission avoidance, improved diagnostic rates, better coordination of care, increased use of anticipatory care plans, reduction in falls, improved management of chronic conditions and reduced need for admission to care homes are amongst the objectives for the whole team. Travel and indemnity included. This role is band 6 or 7 depending on prior experience. Although prior experience working in Frailty or in the community is desirable, it is not essential.

Main duties of the job

The physiotherapist will be predominantly supporting with the development of a community based Ageing Well Clinic, where a multi-professional approach will allow a comprehensive geriatric assessment for identified frail patients. There may be some work with housebound patients and scope for development of strength and balance classes to complement services already available in the locality if the clinic in development identifies additional need. Support and clinical governance will be provided by the lead GP.
