Job summary

We have an exciting opportunity to recruit a Band 7 Respiratory Specialist Nurse to join the Community Respiratory Team within the Weymouth & Portland Locality. This will be a collaborative role working closely with GP Practices, the Local Trust and Secondary Care.

Main duties of the job

The post would suit an experienced Respiratory Nurse Specialist or an individual who wishes to further specialise in the exciting, expanding and challenging field of Respiratory. The successful applicant will work as part of a multidisciplinary respiratory network in the Locality, leading the Community respiratory team, as well as providing education to patients and other health care professionals. The successful applicant will need to have some experience in the field of respiratory, be confident teaching groups of either patients or other health professionals, have excellent interpersonal skills and be innovative, enthusiastic and happy working autonomously.

Job description

Job responsibilities

To plan and provide the coordination of care for patients with respiratory disease in the Weymouth and Portland Locality. To provide a high standard of evidence-based care for respiratory patients, carers and other health professionals, contributing to the effective management of both acute and chronic phases of respiratory problems in the community. This will include:

Providing local leadership to the community respiratory team

Attending and leading monthly community respiratory MDT meetings

Providing sessions of asthma and COPD reviews for complex patients possibly in an MDT style carousel clinic

Providing (in conjunction with Dorset Adult Integrated Respiratory Service (DAIRS)) advice and support for primary care and community staff

Liaising with other teams (eg palliative care, social care)

Supporting practice nurses with patient reviews and spirometry skills (annual review)

Providing support to patients at home during exacerbations

Providing follow up of patients after admission

Ensuring optimum uptake for Pulmonary Rehabilitation

Supporting the development of case finding processes

Providing education to locality health care professionals

Developing and delivering a Respiratory diagnostic hub incorporating clinical assessment, spirometry and FeNO testing

Providing assessment of patients for nebulisers

Support the development of virtual respiratory reviews and group consultations in primary care

Identifying patients for home oxygen therapy and liaising with home oxygen service
