Job summary
Frailty
Nurse required to join an expanding well established community team working to
improve care for our frailer patients. All the practices in Weymouth &
Portland are working together to provide this service.
The Frailty Nurse will join a small team consisting of 2 GPs and 5 Frailty Practitioners to proactively manage patients living in care homes and those who are housebound. Admission avoidance, improved diagnostic rates, better coordination of care, increased use of anticipatory care plans, reduction in falls and improved management of chronic conditions are amongst the objectives.
Although
we are requesting applications for a Band 5 Nurse, for the right individual, we
would consider applications from allied health professionals, experienced
Health Care assistants or higher banded nurses, although the hours available
may be different.
Main duties of the job
We are looking for a committed and enthusiastic Frailty Nurse to join the team to help provide high quality health care for this frail population. This role is anticipated to predominantly support patients living in their own homes, but may involve some visits to care homes.
The
role will be part time, up to 30 hours per week, but the days/times are
negotiable, with no unsociable hours.

Although
prior experience working in Frailty or in the community is desirable, it is not
essential. Phlebotomy skills, ECG taking and recording of basic observations
preferred.

Responsibilities
1. To complete the requested
assessments for registered patients allocated to the team and maintain
appropriate records of treatment given and NHS services provided; When
appropriate, to make a relevant plan, implement this plan and subsequently
evaluate it.
2. Carry out consultations and physical
assessments as requested and maintain high quality records using SystmOne (mobile
working will be facilitated with personal laptop and internet connection).
3. The post-holder will make him/herself available
to undertake visits to care and nursing homes and the housebound as requested, assessing
patients, liaising with the service GPs or registered GP when the service GP(s)
are not available. Attending MDT meetings where possible.
4. Liaise with other members of
community teams, as appropriate to individual patient needs,
5. Arrange for the referral of
patients when it is possible for a community nurse to make the referral,
otherwise defer the need for a referral back to the service GPs.
6. Note any possible issues
with medication compliance noted during any patient contact.
7. Actively work with the wider
MDT by working with, but not exclusively, the community matrons, community nurses,
discharge co-ordinators, elderly care teams at Dorset County Hospital and
community hospitals, dieticians, tissue viability team, CPN, CRT, physio, SALT,
falls prevention services.
8. Maintain appropriate levels
and means of communication i.e. mobile phone, professional email
9. Clinical governance will be
provided by the GP partners and the lead GPs.
10. Training opportunities can
be discussed when appropriate.
