Job summary

The Community Specialist Palliative Care Home Team (SPCHT) at Arthur Rank Hospice Charity supports patients with life limiting conditions across Cambridgeshire. Supported by the wider multi-disciplinary team at the Hospice, the SPCHT ensures that our care isholistic and tailoredto each individuals personal situation.

Due to retirement, we have an opportunity for someone to join this team as a Clinical Nurse Specialist, this is a part time role working 30 hours per week, supporting patients across Cambridgeshire in a Community setting

Our Clinical Nurse Specialists deliver expert care and support to specialist palliative and end of life patients and their families/carers. Working closely with other care providers our team will help manage symptoms, support with medicine management and help access other hospice services, such as complementary therapy.

Main duties of the job

The service runs 7 days a week and colleagues will join us and work on a rota basis to support this.

Main duties will include:

  • To holistically assess, plan, implement and review therapeutic interventions of patients with complex needs
  • Organisation and management of individual caseloads, prioritising and responding to clinical emergencies as required
  • Plan and evaluate programmes of care for patients and carers, undertaking assessments and analysis of physical, psychological and social needs of the patients.
  • Regular communication with other care providers, such as GPs and District Nurses
  • Working as part of the community team to effectively communicate with members of the MDT
  • Working as part of the senior nursing team, providing specialist nursing advice, support and education to other health care professionals and volunteers and deputise for the Matron in their absence as required.

Job responsibilities

To work as an expert clinical nurse specialist within the Arthur Rank Hospice Charity. The post holder will work in all aspects of the team as agreed with the Matron including other clinical services such as Living Well Service, Palliative Hub and Patient and Family Support Team.

  • To provide a high standard of professional conduct and nursing care at all times in accordance with the Nursing and Midwifery Council Code of Professional Practice, demonstrating compassionate leadership in the nursing care of patients within the service.
  • To work as a member of the Arthur Rank Hospice Charity, providing expert knowledge and skills in specialist palliative and end of life care to patients, their families/carers and the Primary Health Care Team.
  • Responsible for leading and developing nurses within the Arthur Rank Hospice Charity, and providing senior nursing support to other areas of the organisation.
  • As a member of the senior nursing team provide specialist nursing advice, support and education to other health care professionals and volunteers and deputise for the Matron in their absence as required.
  • Working across 7-day period as part of a rota.

  • To participate in the Palliative Hub 24/7 rota, providing specialist advice and support to anyone on the Gold Standards register and anyone identified as having a palliative care need and helping to coordinate services.
  • To holistically assess, plan, implement and review therapeutic interventions of patients with complex needs.
  • Plan and evaluate programmes of care for patients and carers .

  • To demonstrate expertise and a firm knowledge base on which to underpin clinical practice when dealing with patients/carers living with a life limiting illness.
  • If qualified as a non-medical prescriber, take responsibility for the assessment and clinical management of palliative care patients known to the service, prescribing medication to end of life patients within scope of clinical expertise. In accordance with NMC, Department of Health and RCN guidelines.
  • As part of the team in community setting effectively communicate with members of the MDT in the provision of care for patients with complex, emotional/psychological, spiritual, physical needs and refer to appropriate disciplines.
  • Identify the needs of bereaved persons and offer psychological support, referring to appropriate bereavement services if required.
  • Plan and deliver teaching sessions for the Charity in conjunction with the Education team
