Job summary

The Hospice at Home service cares for patients across Cambridgeshire who choose to remain in their own home in their last weeks of life. We are holistic in our approach and the care is tailored to each persons specific needs. The compassionate team of Nurses and Healthcare Assistants work closely with District Nursing Teams and GP's to provide practical and holistic care supporting patients and their families in their homes.

We now have an opportunity to join us as aRegistered Nurse in a permanent role, based from our Hospice in Cambridge,but working across Cambridgeshire. This is a full time role, working 37.5 hours per week.

Our nursing teams work long shifts (07.00 19.00, 08.00 20.00) or a mixture of shorter shifts.

Main duties of the job

Successful applicants will have excellent communication skills and interpersonal skills and will enjoy having quality time to provide exceptional care to our patients and their loved ones in their homes

The service provides care 7 days a week across Cambridgeshire and staff are rostered to provide this coverage. Whilst the rosters are created 5- 8 weeks in advance, there may be exceptional circumstances in which the job holder will need to be flexible to change a scheduled rota at short notice in times of clinical staffing crisis.

Colleagues working within our Hospice at Home (HaH) team cover a wide geographical area, from Tydd St Giles in the North of the county down to Saffron Walden in the South, and everywhere in between. Although allocated to either the South team (Cambridge)or North team (Wisbech)base, colleagues work flexibly as one team in order to meet patient needs wherever they may be. For this reason, we are pleased to offer mileage expense rates in excess of HMRC as well as a lease car option. Whilst every effort is made to allocate patients close to where you live, this is not always possible we ask all of our colleagues to be flexible and attend patients wherever they are located.

The Hospice at Home service cares for patients across Cambridgeshire who choose to remain in their own home in their last weeks of life. Job Purpose:

To work within a team to assist in the responsive delivery of nursing palliative and end of life care, ensuring the highest standard of care are received by patients and their families in their own home.

This role will work within the Hospice at Home team to assist in the responsive delivery of nursing palliative and end of life care, ensuring the highest standards of care are received by patients and their families in their own home.

Main duties to include:

  • Using a holistic approach and in partnership with the district nurse, patient & family to assess, plan, implement and evaluate individual packages of care.
  • Assess, monitor and evaluate symptom management
  • Assisting in the effective orientation and mentoring of new staff and clinical placements to the team.
  • Support individuals and carers to cope with their emotional, psychological and spiritual well-being

The service provides care 7 days a week across Cambridgeshire and staff are rostered to provide this coverage. Whilst the rosters are created 5- 8 weeks in advance, there may be exceptional circumstances in which the job holder will need to be flexible to change a scheduled rota at short notice in times of clinical staffing crisis.

To be successful in this role, candidates must be a NMC Registered Nurse and have post registration experience within palliative care. It is also required to have a knowledge of a holistic approach to care giving and be sensitive to the needs of patients and families.
