We have an opportunity for a Community Sister/Charge Nurse to join our Hospice at Home team, providing care to patients in their own homes. We have a full time role available based at Arthur Rank Hospice in Cambridge, covering Ely, Cambridge City and South Cambridgeshire.

As a member of the Hospice at Home team you will further enhance the high-quality service, providing expert knowledge and skills in the provision of palliative and end of life care to patients, their families/ carers and the Primary Health Care Team. You will support through line management responsibilities band 2, 3 and 5 collegues.

Main duties of the job

The successful candidate has to be: Registered General Nurse (Adult), have relevant post registration experience at Band 5 experience or qualification as well as a knowledge and understanding of palliative and end of life care. It is also a requirement to have experience of teaching and multi-professional working.

You will have to be able to work flexibly across 7 days per week, willing to travel between sites within community area (South Cambs) and have full clean UK driving licence and access to own car.

Job Purpose:

Working as a member of the Arthur Rank Hospice, specifically based within the Hospice at Home function, providing expert knowledge and skills in the provision of palliative and end of life care to patients, their families/ carers and the Primary Health Care Team. To support through line management responsibilities band 2, 3 and 5 colleagues.

Main Responsibilities:

Facilitate the holistic assessment, planned treatment and evaluation of patient centered care

Promote, develop and apply principles of Specialist Palliative Care

Be responsible for ensuring high standards of care are achieved and maintained according to Arthur Rank Hospice Standards and Policies

Liaise closely with all members of the multi-disciplinary team within the charity and externally within the community.

Act as an expert role model and manage time effectively, supporting the Hospice at Home Healthcare Assistants, acting as line manager and mentor.

Coordinate, manage and allocate HCA teams to deliver Hospice at home day care service.

To work flexibly across the community palliative caseload with Community Palliative Clinical Nurse Specialist and assess patients at home if required.

Plan meetings and continually assess the needs of the team of Hospice at Home staff.

Promote and contribute to the development of research based practices

Promote and contribute to innovations in clinical practice

Continuously evaluate and develop service

Encourage a learning culture through close involvement in continuing education of self and others. Support Hospice at Home team and contribute to the Education program

Attend any relevant meetings as the community Hospice at Home representative

Assist in developing a climate that embraces Quality, Audit and formal service review

Facilitate, with the support of the Matron, the professional development of their team members through the use of the objective setting and review process

Take an active role in the recruitment, selection and orientation of new staff to the team

Act as supervisor/mentor to visiting students and staff undertaking educational courses

Attend statutory training with respect to moving and handling, fire safety and others as required

Demonstrate commitment to the clinical supervision process

Assist in maintaining adequate supplies of clinical equipment within the agreed budget and ensure resources are used effectively

Liaise very closely with Band 7 team lead and deputise in their absence

Exercise leadership skills in the development and support of team members through regular communication and the organisation of team meetings

Liaise with multi-professional agencies, either outside or within the Trust to maintain effective working relationships

Be prepared to work out of hours to support and supervise new and existing staff

Ability to work flexibly across a 7 day a week service.

Ability to work flexibly within the Arthur Rank Hospice and the Alan Hudson Day Treatment Centre.

Ability to work under pressure and in difficult situations.

Ability to work well within a team and display self-motivation, organisation skills.
