Job summary

We are seeking to recruit a Pharmacy Technician to join our multidisciplinary Primary Care Network (PCN) team, working closely with clinical pharmacists to support high-quality patient care.

Clinical Pharmacy in General Practice is part of an exciting transformation programme, developing a new model of care across Southport and Formby PCN. This approach aligns with national priorities, including the Five Year Forward View and GP Forward View, ensuring pharmacy professionals play a key role in helping patients stay safe, well, and out of hospital, while reducing pressures on general practice.

This is a full-time, 11-month fixed-term position (maternity cover) based at Southport & Formby Health. Part-time applications will also be considered.

Main duties of the job

Working across PCN practices, the post holder will support clinical pharmacists by undertaking clinical reviews of patient records, identifying and referring or flagging areas of concern. The role also includes counselling patients, reviewing and accurately recording medications, and participating in audits, with a particular focus on patients prescribed high-risk drugs. This work is central to improving the quality of care and patient outcomes.

The role is pivotal in enhancing both care quality and operational efficiency, and therefore requires a motivated individual with a passion for delivering excellent service within general practice. The successful candidate will be supported in developing their role throughout the 11-month contract period.

This includes supporting patients to make informed decisions about their health and connecting them with a range of statutory and voluntary services to ensure they can access the right care at the right time.

Carry out medicines optimisation tasks including effective medicine administration, supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.

Support the Clinical Pharmacist in preparing for Structured Medication Reviews (SMR)

Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.

Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.

Support the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed.

Develop relationships with other pharmacy professionals and members of the multi-disciplinary team to support integration across health and social care including primary care, community pharmacy, secondary care, and mental health.

Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.

Support the PCN to deliver on QIPP agenda, Local Quality Contract (LQC), Quality and Outcomes Framework (QOF) and other locally commissioned enhanced services

Support the PCN in reviewing and developing practice policies for CQC requirements

Skills and Qualifications

The post holder meets the specific qualification and training requirements as specified by the GPhC criteria to register as a Pharmacy Technician

Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice

Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate

Able to follow legal, ethical, professional and organisational policies, procedures and codes of conduct

Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines. NB: it is anticipated level of qualification held may vary according to the level of position and the components of the role being carried out

Demonstrates self-development through continuous professional development activity; working alongside senior clinical pharmacist to identifying areas to develop

Demonstrate understanding of the pharmacy role in governance and is able to implement this appropriately within the workplace.

Demonstrates understanding of principles of research governance.
